شناسايي تعيين گرهاي كيفيت روابط زوجي اثرگذار بر بيماري لوپوس در زنان: يك مطالعه كيفي

Identifying the influential determinants of marital relationship quality on lupus diseases in women: A qualitative research

بيماري لوپوس يك بيماري خود ايمن است كه علت ابتلا و عود آن از منظر پزشكي مشخص و قابل پيش‌بيني نيست، زيربناي اين بيماري نابهنجاري در فرآيند التهاب است. التهاب نيز اولين پاسخ سيستم ايمني به محرك مضر است. ادراك حمايت اجتماعي و رضايت زناشويي بر سيستم ايمني و التهاب اثرگذار است هدف: پژوهش حاضر با هدف يافتن تعيين ­گرهاي كيفيت روابط زناشويي مؤثر بر روند بيماري لوپوس انجام گرفت. روش: در اين پژوهش از رويكردي كيفي به شيوه پديدارشناسي تفسيري استفاده شد. بدين منظور با استفاده از روش مصاحبه نيمه ساختاريافته، تجربه زيسته 15 بيمار زن مبتلا به لوپوس كه به‌صورت ملاك محور انتخاب شدند، با استفاده از روش ديكلمن، آلن و تانر (1989) و نرم ­افزار MAX-QUDA-10 مورد واكاوي قرار گرفت. يافته ­ها: پس از پياده‌سازي مصاحبه‌ها و تحليل عميق داده‌ها 53 نشانگر، 17 ملاك و 5 عامل اصلي شامل " ويژگي‌هاي شخصيتي همسر"، " سبك تعاملي با همسر"، " هيجانات ادراك‌شده در رابطه‌ي زوجي"، " حمايت ادراك‌شده از سوي همسر در برابر بيماري" و " تنش گرهاي موقعيتي" شناسايي شد. نتيجه‌گيري: با لحاظ نمودن تعيين ­گرهاي كيفيت روابط زوجي كه بر روند اين بيماري و حتي پاسخ‌دهي بيمار به درمان اثرگذار است، گام مهمي در بهبود اين بيماري مي­ توان برداشت.

Lupus is an autoimmune disease which cause and flares up are not medically predictable from a medical point of view. Inflammation is also the immune system's first response to a harmful stimulus. Perceptions of social support and marital satisfaction affect the immune system and inflammation. Aims: The general purpose of this study is to identify methods of stabilizing career well-being, in other words, identifying resources that directly or indirectly increase well-being and effectively Prevent adaptation to organizational stimuli. Methods: In this study, a qualitative approach to interpretive phenomenology was used. For this purpose, using the semi-structured interview method, the living experience of 15 patients with lupus who were selected as a criterion was analyzed using Diekelman (1989) method. Results: After conducting interviews and in-depth data analysis, 53 indicators, 17 criteria and 5 main factors including "spouse personality traits", "interactive style with spouse", "perceived emotions in a couple relationship", " Perceived support by the spouse in the face of illness." and "situational stressor". Conclusion: By considering the determinants of couple relationships quality which affect the course of this disease and even the patient's response to treatment, an important step can be taken to improve this disease.
