تدوين مدل سياست گذاري براي توانمندسازي بيمه شدگان بيمه سلامت ايران

Developing a Policymaking Model for Empowering the Insured of the Iran Health Insurance Organization

مقدمه: پژوهش حاضر به دنبال طراحي مدل توانمندسازي بيمه شدگان بيمه سلامت است تا طي آن بيمه شدگان بتوانند به سطح قابل قبولي از توانمندي دست پيدا كنند. دليل انتخاب اين گروه از بيمه شدگان، جمعيت زياد و قابل ملاحظه آنان در كشور است؛ به گونه اي كه اكنون حدود 50 درصد از جمعيت ايران را شامل مي‌شوند. روش بررسي: رويكرد روش‌شناختي اين پژوهش كيفي و از نوع نظريه مبنايي، فنون داده يابي مصاحبه عميق و نيمه ساختاريافته است. براي داده‌كاوي از روش كدگذاري نظري استفاده شد كه دستيابي به مقولات اصلي و فرعي در راستاي تنظيم مدل نظري مدنظر است. تعداد نمونه 15 نفر از متخصصان و خبرگان حوزه بيمه سلامت است كه به صورت هدفمند انتخاب شده اند. يافته ها: نتايج پژوهش نشان مي دهد كه پديده محوري توانمندي اجتماع‌محور است كه مبتني بر شرايط علّي، شرايط زمينه اي، شرايط مداخله‌كننده و راهبردها به دست آمده است. مهم ترين راهبردها حق دسترسي بيمه شدگان به خدمات و فرصت هاي بيمه اي است. در بحث از شرايط زمينه اي مؤلّفه هاي مهمي چون عامليت، احساس برابري و شمول (ادغام) و ظرفيت سازي مورد تأكيد است. همچنين يافته ها نشان مي دهد مؤلفه هايي چون حفظ حقوق بيمه شدگان، منابع توانمندساز و كارآمد و كيفي بودن خدمات بيمه اي به عنوان شرايط علّي مورد توجه هستند. نتيجه‌گيري: بنا بر يافته هاي پژوهش حاضر 3 مقوله اصلي آموزش و اطلاع‌رساني، مشاركت اجتماعي و حمايت اجتماعي به عنوان شرايط مداخله گر مطرح شده اند. تكميل كننده زنجيره مدل پارادايمي پژوهش حاضر مبتني بر روش نظريه داده بنياد پيامدها هستند كه توانمندسازي را در 4 بخش عمده توانمندي اجتماعي، توانمندي فرهنگي، توانمندي اقتصادي و توانمندي رواني بيمه شدگان مورد واكاوي قرار داده است

Introduction: The present study seeks to design a health insurance empowerment model so that the insured can achieve an acceptable level of capabilities. The reason for choosing this group of insured persons is their large and considerable population in the country, which now include about 50% of Iran’s population. Methods: The methodological approach of this research is qualitative and based of the grounded theory, deep and semi-structured interview data retrieval techniques. For data mining, the theoretical coding method was used, which aims to achieve the main and secondary categories in line with the theoretical model setting. The sample number is 15 health insurance experts who were selected purposefully. Results: The results show that the central phenomenon is community-oriented empowerment, which is based on causal conditions, contextual conditions, intervening conditions and strategies. The most important strategies are the right of the insured to access insurance services and opportunities. In the discussion of background conditions, important elements such as agency, sense of equality and inclusion (integration) and capacity building are emphasized. Also, the findings show that components such as maintaining the rights of the insured, enabling resources and the efficiency and quality of insurance services are considered as causal conditions. Conclusion: According to the findings of the present study, three main categories of education and information, social participation and social support have been proposed as intervening conditions. Completing the chain of the paradigm model of the present study is based on the data theory method of the Consequences Foundation, which analysed the empowerment in four main sections of social empowerment, cultural empowerment, economic empowerment and psychological empowerment of the insured.
