EFFECTIVNESS OF APPLYING CONTINUOUS CARE MODEL ON QUALITY OF LIFE LEVEL IN DIABETIC PATIENTS

Backgrounds & Aims: Diabetes mellitus is a common chronic medical condition associated with significant global morbidity, mortality and costs. Patients show an increased prevalence of various psychiatric and somatic disorders, mainly depressive, anxiety disorders, Physical inactivity and obesity. This problems decreases quality of life in diabetes, so enhancement of QOL in diabetic patients is the major aim of diabetes control. This study was carried out in order to determine the effect of applying continuous care model on quality of lifeʹs level in diabetic patients. Materials & Methods: This is a Quasi-experimental study which is done in Uremiaʹs Imam Reza hospital. 74 DM type II patients were selected with random allocation technique and assigned case &control groups for the purpose of this research. Data collection tools include: demographic informationʹs & a quality of life questionnaire. Results: The SPSS software outputs (T Test) showed: the mean of Q.O.L in case group changes from 98 to 111. There is a relationship between applying continuous care model & quality of lifeʹs level in diabetic patients. Conclusion: Considering this findingʹs, research hypothesis that applying continuous care model enhanced quality of life levels in DM patients was approved. Therefore we propose to applying continues care model for diabetic patients.
