A STUDY OF THE RATE OF INCIDENCE OF FETAL MALPRESENTA TIONS, RELATED FACTORS, AND COMPLICATIONS

Malpresentations are major causes of perinatal and maternal mortality and morbidity.To determine the prevalence of malpresentation in parturients and evaluate some of their related factors, management and outcomes; a cross sectional study was conducted on 5102 parturients attending the two major obstetric centers of Kerman city , consecutively. All singleton births with gestational age more than 28 weeks were included in the study.The prevalence of malpresentation was 4.31%, 3.86% in the breech presentation, 0.37% in the transverse lie and 0.08% in the face presentation.The mean gravidity was significantly higher in the face presentation and prematurity was more frequent in the breech presentation (P<0.01) Neonatal weight was not significantly different among various malpresentations.Cesarean section rate differred among various groups. The highest rate occurred in the face and the lowest in the cephalic presentation (p<0.001). Early neonatal death was significantly higher in the breech presentations .With respect to the frequency of malpresentations in Kerman city, physicians must focus on early detection and suitable management in order to reduce the maternal morbidity and perinatal mortality rates.
