EVALUATION OF THE THERAPEUTIC EFFECT OF PHENYTION IN ASTHMA TIC PATIENTS

Introduction: Asthma is an airway inflammatory disease pertaining to specific cells resulting in H.R of the airway and eventually, the asthma attack. Phenytoin with different mechanisms affects the edema and the airway. Studies have been done in this field, so this research was done to confirm or refund the claim. Methods : The research has been performed on 190 asthmatic patients (104 male, 80 female). The patients were divided into two groups (111 people and 2-79 people) and examined monthly which included physical examination , observation and spirometry According to the severity of the disease , they were prescribed beclotide - atrovent (Inhaler), theophyline , (tab) and inhaler salbutamol respectively . The medicines were prescribed according to the severity of the disease. With dereasing severity of sickness , the dosage of steroids were reduced, and subsequently theophyline & atrovent were reduced successively. Asthma attacks, maximum steps the patient could climb, night awakening as result of dyspnea and spirometry were recorded. Group 1 was advised to take 100 mg. Phenytoin (capsule) twice a day and Ggroup 2 multi vitamin (capsule) twice a day. The treatment lasted for 12 months, and then the patients in Group one took multivitamins for 18 months. Results : Applying the Mann Whitney Wilcox analysis , the following results of the effect of phenytoin were obtained. 1- On reduction of dosage of prednisolone P.V = 0.001. 2- On night awakening P.V = 0.05 . 3- On Number of the asthma attacks P.V = 0.437. 4- On increase in FEV1 P.V = 0.01. Conclusion : Taking into consideration the results of this study and the numerous side effects of phenytoin, it is advisable to prescribe phenytoin for those asthmatics suffering from epilepsy or those patients requiring high dosage of steroids, but unable to tolerate them.
