The Methodology of Safety Role in Cost Control; A Case Study of Chlorine Unloading Operation

By defining accident, the flow chart of accident was described and the direct and indirect cost of accident was listed. The scenario of a simple accident in which a worker who was carrying some files & documents, fell down in the corridor, was given, The total cost of this accident which was about $5000, was notnoted. As a case study the failure of chlorine hose during unloading of a chlorine tanker into a pressurized storage vessel, which is a routine job in the cosmetic and household detergent producing industries, was analyzed. For this event a fault tree was built up. By considering the failure rates of basic events, the failure rate for the top event was calculated to be 4.3X10-3 per year. A cost effectiveness analysis was followed by recommendations of a control measure as an interlock system in one part of the tree. This safety lock would not let the driver move tanker before disconnecting the hose. Application of this control measure in the fault tree decreased the failure rate of top event to 3.1X10-4 per year. This means that the occurrence of one accident in every 2325 years (before application of control measure) will change to one accident in every 3225 years.Assuming the total amount of 500,000,000 Rials as the cost of the accident, understandy the annual cost will be 215,053 Rials. After application of the proposed control measure the annual cost will decrease to 155, 034 Rials. This means a 60, 019 Rials sailing per year due to the proposed control measure.
