Introduction: Cytomegalovirus is considered as a risk factor in kidney transplant recipients. Usuallyʹ, infection with this virus leads to rejection of the transplant organ and may results in patients death. Monitoring the condition of the patients infected with this virus is necessary for such patients. However several laboratory diagnostic methods are available to assist the physicians for prevention or treatment of active cytomegalovirus infections, but some of these methods are not sufficiently reliable. Methods:This study was carried out as a descriptive and cross-sectional one from 1380 to 1381 on 50 kidney transplant recipients. Two blood samples were collected from each patient. The first sample (6ml) with anticoagulant factor was evaluated by antigenemia method to detect pp65 antigen and the second sample (3m1) without anticoagulant factor by ELISA method. Results were analyzed by chi-square and relative prevalence statistical methods. Results : The results showed that 10% were positive by antigenemia method. All of the patients were positive by ELISA-IgG, 84% by IgGB and 6% by IgM . The difference between antigenemia and Elisa for diagnosis of active cytomegalovirus infection was statistically significant (p<0.05). Age of patients had effect on the result (p<0.05), but there was no such correlation with the patients sex. Conclusion : The present study showed that Elisa is not a reliable test for diagnosis of active cytomegalovirus infections and it is suggested that patients who are ELISA positive should be controlled with antigenemia technique and administered antiviral drugs on basis of positive antigenemia test results.
