CURRENT PERCEPTION THRESHOLD IN DIABETIC NEUROPA THY AND ITS COMPARISON WITH NERVE CONDUCTION STUDY

Prevalence of diabetic neuropathy is not fully determined and its prevalence is reported between 5 and 65% in different studies . Conventional nerve conduction studies (NCS) have shortcomings in the evaluation of all nerve fibers and nerve function. in this regard , we studied current perception threshold (CPT) and compared it with NCS.A descriptive cross sectional study was done on 47 consecutive diabetic patients and 50 normal persons at the department of Physical Medicine and Rehabilitation in Nemazi hospital during winter 2002. Of the 47 diabetic patients , 25 (53 %) had abnormal NCS , 31 (66 % ) had abnormal CPT (p = 0.12) and 42 ( 89 %) had some abnormalities in at least one of these two methods . Of the 9 patients with less than 1 year history of diabetes , only I (1 1 %) had abnormal NCS , while 7 ( 77 % ) revealed abnormal CPT (p = 0.19). CPT and NCS are not preferable over each other for the diagnosis of diabetic neuropathy, but considering both methods in adjunction increases their diagnostic sensitivity. It seems that CPT is more sensitive than NCS in diagnoses of early stag es of diabetic neuropathy.
