Varicocele is the most common correctable cause of infertility in men that is detected in 20-40% of infertile men. According to literature, varicocelectomy could lead to pregnancy and improvement in spermograms in 60% and 83%, respectively.The aim of this study is to evaluate Comparison of the variations in the T.M.F.S following varicocelectomy with respect to age, grade and unilat- erality or bilaterality of varicocele.This clinical trial was performed on 150 infertile men since 2003 to 2006, who were operated on, for varicocele. In our study, the patients were divided into 9 groups based on the grade of varicocele, unilaterality or bilaterality. Then the patients were classified into two groups with respect to their age, i.e. younger than 30 years of age or older.Spermograms were obtained before and 4-6 months after operation. The results were analyzed by Variance-test, T-test and chi-square.In our study, 53.3% of patients had unilateral versus 46.7% of bilateral varico- cele.The median age was 30. Ninety-three patients were younger than 30 while 57 were older than 30 years old. In 86% of patients, spermogram improved. There was no significant difference in spermograms in various groups of the study (P-value=0.871).Although in both age group, spermogram improvement were significant (P-value=0.0001), but the result of the operation was better in the older group (P-value=0.041).Due to the significant effect of varicocelectomy on fertility, this operation is recommended irrespective of the grade and age of the patients.
