Risk Factors and Perinatal Outcomes Associated with Umbilical Cord Prolapse

Umbilical cord prolapse is a serious complication in obstetric emergency. It can cause fetal death and increase the risks during pregnancy. It is crucial for the staffs working in obstetric emergency units to be aware of the risk factors related to umbilical cord prolapse. The aim of our study was to evaluate the incidence, risk factors and perinatal outcomes of pregnancies complicated by umbilical cord prolapse.Birth records of 76 cases with umbilical cord prolapse and 760 randomly selected controls were retrospectively reviewed. A statistical analysis was performed, using logistic regression models.Prolapse of the umbilical cord complicated 0.32ʹ/. (n=76) of all deliveries included in the study (n=23818). Multiparity was more common in patients with umbilical cord prolapse (88.2/., PO.OOOl). Umbilical cord prolapse was found to occur in breech presentation for 17 cases (22.4/.) and in transverse presentation for 3 of the cases (9.2 /.). The occurrence of breech presentation among the control cases was 1.2 /. while that of the transverse lie was 0.9ʹ/.. Fetuses with umbilical cord prolapse had lower fetal weight; particularly, the fetal weight of less than 2500 g was a significant risk factor (9.2/., P<0.05). We also found that the premature rupture of membranes (PROM) (OR=0.92; 957, CI-0.81 - 0.99), and polyhydramnios (OR- 0.91; 95 /, CI-0.84 - 0.97) were among the risk factors for umbilical cord prolapse. The newborns delivered after umbilical cord pro-lapse were graded lower Apgar scores of less than 7 in 5 minutes (9.2/, PO.OOOl).Abnormal fetal presentation, multiparity, lowbirth weight, prematurity, polyhydramnios, and premature rupture of membranes (PROM), are the risk factors for umbilical cord prolapse.
