Presence of ESBL in Klebsiella pneumoniae strains isolated from clinical samples in Shariaty Hospital in Tehran

The over use of antibiotics has been led to introduction of multi drug resistant klebsiella pneumoniae strains all around the world in the last decades. The production of extended spectrum B-lactamase (ESBL) is one of the mechanisms of resistance to B-lactam antibiotics among gram negative bacteria specially k.pneumoniae. The aim of this research was to determine resistance pattern of k.pneumoniae isolated from clinical samples to the B-lactam antibiotics and study of presence of ESBLs in these isolates. The 40 isolates of K. pneumoniae were collected from clinical samples of Shariaty hospital in Tehran and antibiogram of isolates was determined by disk diffusion method. The MICS for Ceftazidime antibiotic were also determined using broth dilution assay and the presences of ESBL in isolated strains were detected by disc diffusion method. All strains were found to be sensitive to Imipenem. The rate of resistance to Cefotaxime, Ceftazidime and Piperacillin were 35%, 37% and 57% respectively. The 42% of the ESBLs positive strains showed MIC more than 32 Mg/ml. The results showed that due to high reistance to B -lactam antibiotics and the high rate of ESBLs in the isolated strains, use of B-lactam antibiotics especially the third generation of Cephalosporins must be limited.
