Etiology and pathogenesis of antisperm antibody

مقدمه: آنتي اسپرم آنتي بادي ها (ASA) در مردان و زنان ايجاد مي شوند و ممكن است به طور قابل توجهي باروري را معيوب نمايند. در اين مورد، بيضه يك مكان امن ايمونولوژيك است كه در آنجا آنتي ژن هاي سلول ژرم از حمله خودايمني محافظت مي شوند. با اين وجود، در نتيجه شكسته شدن سد خوني- بيضه اي حاصل از آسيب بيضه اي يا در نتيجه ضربه به اپيديديم يا وازدفران اغلب پروتيين هاي بيضه اي در طول برخوردهاي ايمونولوژيكي منجر به تشكيل ASA در سرم خون، پلاسماي سمن يا سطح غشا اسپرم، اتوآنتي ژنيك مي شوند. همچنين گزارش شده است كه ASA با التهاب، كريپتوركيديسم، واريكوسل و مداخله جراحي در اندام هاي تناسلي ارتباط دارد. ASA ممكن است در عملكردهاي مختلف اسپرم كه براي روندهاي باروري ضروري هستند ايجاد اختلال نمايد. اين مقاله مروري به افزايش فهم ما از مكانيسم هاي اختصاصي كه پاسخ خودايمني به اسپرم را برمي انگيزند و پاتوژنز ASA كه به ناباروري با واسطه آنتي بادي منجر مي گردد كمك مي كند

Background :Antisperm antibodies (ASA) occur in men and women and may significantly impair fertility. In this case, the testis is an immunologically privileged site where germ cell antigens are protected from autoimmune attack. However, due to disruption of the blood-testis barrier occurring from testicular injury, or as a consequence of trauma to the epididymis or vas deferens many testicular proteins get autoantigenic during immunological challenges resulting in the formation of ASA in the blood serum, seminal plasma or located on the sperm membrane. ASA have also been reported to be associated with inflammation, cryptorchidism, varicocele and surgical intervention in the genital organs. ASA may interfere with different sperm functions, which are essential for the fertilization process.This review article will help to increase our understanding of the specific mechanisms that elicit the autoimmune response to sperm and of the pathogenesis of ASA that leads to an antibody-mediated infertility.
