ارزيابي دستگاه Spiral tomography در پيش بيني موقعيت كف سينوس ماگزيلاري

Assessing spiral tomographic technique to locate the inferior border of maxillary sinus cortex

خلاصه سابقه وهدف: در اعمال جراحي دقيق (مانند جراحي هاي ايمپلنت دنداني) تعيين موقعيت دقيق تحتاني ترين بوردر كورتكس كف سينوس ماگزيلاري براي اجتناب ازسوراخ شدن كف سينوس از اهميت ويژه اي برخوردار است. هدف از مطالعه حاضر؛ ارزيابي دستگاه Spiral tomography در تعيين موقعيت تحتاني ترين بردر كورتكس كف سينوس ماگزيلاري مي باشد. مواد و روش ها: در اين مطالعه توموگرافي اسپيرال در نواحي مشخص شده به وسيله گوتاپركا در هر ماگزيلا خشك توسط دستگاه Cranex tome multifunctional x-ray ساخت كارخانه Sordex فنلاند، انجام گرفت. سپس اسكال ها در محل هاي مشخص شده توسط دريل مخصوص برش خوردند. اندازه گيري ها بر روي توموگرام ( A,B) و ماگزيلا خشك (A´,B´) براي تك تك نمونه ها صورت گرفت. يافته ها: در اندازه گيري فواصل افقي ميزان خطاي اندازه گيري بين -0/56mm تا +0/49mm مي باشد. به عبارت ديگر100? نمونه‌ها در محدوده قابل قبول 1± و 4/49? نمونه ها در محدوده قابل قبول 0/5± بود. در اندازه گيري فواصل عمودي به صورت خام ميزان خطا بين 3/36mmتا9/81mm بيش از اندازه واقعي بود. با توجه به اين مسيله معادله رگرسيوني با اعتبار برابر 0/976 بين اندازه فواصل عمودي به دست آمده و gold standard برقرار شد كه با احتساب اين معادله ميزان خطا بين 1/2mm_تا +1/4mm رسيد و83/3 ? نمونه ها در حد فاصل خطاي mm1± و 66/7 ? نمونه ها در محدوده خطاي 0/5mm± قرار گرفتند. نتيجه گيري: اسپيرال توموگرافي با دستگاهunit Cranex tome multifunctionalاطلاعات دقيقي در طرح ريزي درمان پيش از جراحي هاي ايمپلنت در ماگزيلا را فراهم مي كند.

Abstract: Background and aim: for certain surgical procedures (e.g. placement of implants) , an accurate localization of the lowest border of the maxillary sinus floor cortex is important to avoid perforating maxillary sinus floor . the aim of the present study was to evaluate , the accuracy of spiral tomography in the localization of the lowest broder of the maxillary sinus floor cortex. Material and Methods: using cranex Tome multifunctional unit (Orion corporation Sordex, Finland), tomographic slices were taken at marked sires by gutta percha in each dried maxilla. The following measurements were performed both on the tomograms (A,B) and the bone sections (A,B) at these sites . Results: In horizontal measurements , the range of difference is between 0.56 mm and + 0.49 mm, on the words 100% of case are in ±1mm and 94.4% of cases are in ±0.5 mm (over estimated and under estimated ) . In vertical measurements , the range of crude difference was overestimated between 3.36 mm and 9.81 mm . Considering this regression equation with validity B score of 0.976 was established , the rate of difference was reached between -1.2 mm and +1.4 mm and 83.3% of cases are in ±1mm and 66.7% of cases are in ±0.5 m . Conclusion: spiral tomography with the Cranex Tome multifunctional x-ray unit provides accurate placement information and sufficient details for preoperative planning of implant in the posterior region of maxilla .
