Comparison of the health indicators of the state health sites and health indicators which have been outsourced to private sector healt facility south of Tehran, Iran in 2009

زمينه و هدف: با گذشت بيش از 4 سال از واگذاري خدمات بهداشتي در قالب پايگاه‌‌هاي بهداشتي شهري در دانشگاه علوم پزشكي و خدمات بهداشتي درماني تهران (مطابق ماده 88 قانون تنظيم بخشي از مقررات مالي دولت)، سنجش ميزان دستيابي به اهداف ‌اين راهبرد ضروري به نظر مي‌رسيد. هدف از ‌اين مطالعه بررسي موضوع واگذاري پايگاه‌‌هاي بهداشتي به بخش خصوصي از منظر شاخص‌‌هاي بهداشتي در قياس با پايگاه‌‌هاي بهداشتي مشابه دولتي مي‌باشد. روش بررسي: در ‌اين مطالعه توصيفي-تحليلي دو پايگاه از پايگاه‌‌هاي غير‌ دولتي با دو پايگاه از پايگاه‌‌هاي دولتي انتخاب و شاخص‌‌هاي بهداشتي در آنها بررسي گرديد. يافته‌‌ها مبتني بر اطلاعات موجود در واحد‌‌هاي ارايه خدمات بوده و جهت ارايه داده‌ها و تحليل آنها از آمار توصيفي و آزمون كاي دو در نرم افزار Mini tab استفاده شد. يافته ها: در ميان طيف مراقبتي كودكان زير 6 سال تنها در دو مقطع سني زير 2 ماه و 5 سال درصد پوشش مراقبت در بخش خصوصي بالاتر بود (05/0P < ). در ساير مقاطع سني شامل زير يكسال، 2-1 سال، 3 سال و 4 سال درصد پوشش در بخش خصوصي پايين تر بود (05/0P < ). ميزان پوشش تنظيم خانواده و واكسيناسيون كودكان زير يكسال نيز در پايگاه‌‌هاي بهداشتي واگذار شده كمتر بود (05/0P < ). ولي ميزان پوشش مراقبت مادران باردار در بخش خصوصي بالاتر از بخش دولتي بود (05/0P < ). نتيحه گيري: ارايه خدمات مراقبت هاي بهداشتي پايگاه هاي واگذار شده به بخش خصوصي در زمينه مراقبت كودكان زير 6 سال، مراقبت زنان واجد شرايط تنظيم خانواده و واكسيناسيون كودكان زير يكسال نيازمند ارتقا مي باشد. لذا با توجه به اينكه برون سپاري با هدف دستيابي به عملكرد مطلوب و بهبود ارايه خدمات صورت گرفته است، لازم است جهت تحقق اين اهداف گام هاي موثرتري برداشته شود.

Background and aim: Over more than four years of the outsource of health services in urban health posts in the Tehran University of Medical Sciences and Health Services in accordance with Article 88 of the Act is part of the government financial regulations, assess the extent necessary to achieve the objectives of the strategy seemed to be. The purpose of this study was to examine the issue of outsource of health facilities to the private sector from the perspective of health indicators compared to government health posts are the same. Methods: These analytical study 2 sites from the number of non-governmental sites were chosen and checked. Its health indicators are addressed. Form is used to collect data from the findings of these paper-based information services units and data were analyzed using descriptive statistics and ?2 in the minitab software. Results: Among the spectrum of care for children under 6 years only two students under the age of 2 months and 5 years, the coverage of private section was more obvious (P < 0.05). In other sections under the age of one year, 1-2 years old, 3 and 4 years old the coverage of private section was low (P < 0.05). In the field of family planning and vaccination coverage rates was lower in the health data base (P < 0.05). Coverage of pregnant women in the private section was higher than the government sections. Conclusion: health care services bases leased to the private sector in caring for children under 6 women eligible for family planning care and children under one year vaccination،need to be upgraded. Financial Regulations with the aim of achieving optimal performance is to improve service delivery, need for more effective steps be taken to achieve these goals
