بررسي تطبيقي سير تكامل و ساختار سيستم هاي نامگذاري نظام يافته پزشكي

Comparative study of evolution process and structure of systematized nomenclature of medicine in America, England and Australia countries

زمينه و هدف : پيشقدمي هاي پيشرفته و تحقيقي اخير در پرونده هاي الكترونيك سلامت بر كاربرد اصطلاح شناسي هاي رسمي و استاندارد تاكيد دارند. سيستم نامگذاري نظام يافته پزشكي-اصطلاحات باليني (SNOMED-CT) يكي از توسعه يافته ترين اصطلاح شناسي هاي اين نوع است. اين سيستم بر پايه مفهوم است و ايجاد زباني مشترك، يكپارچگي را در گردآوري و تبادل داده هاي باليني ارايه كنندگان خدمات بوجود مي آورد و ساختار لازم براي توسعه سيستم هاي EHR را تسهيل مي نمايد. روش پژوهش: در اين پژوهش دانش بررسي سيستم هاي نامگذاري نظام يافته پزشكي از روش توصيفي و در بخش نتيجه گيري از مقايسه و تطبيق استفاده شده است. گرد آوري اطلاعات از طريق بررسي كتب ، مقالات ، نشريات خارجي و اينترنت مي باشد. اطلاعات هر كشور با توجه به اهداف مورد نظر با هم مقايسه شده و نتايج حاصله ارايه شده است. يافته ها: يافته هاي حاصل از روند تكامل اين سيستم عبارتند از اينكه كشور آمريكا و انگلستان،(SNOMED-CT) را با كاربرد در همه فعاليتهاي باليني بعنوان اصطلاح شناسي باليني رفرنس اقتباس نموده و كشور استراليا تحت ارزيابي اين سيستم است و در حال حاضر از ويرايش هاي قديمي تر تنها در كاربردهاي پاتولوژيك استفاده مي نمايد. بررسي ساختار آن نشان داده كه پيچيده و بر پايه مفهوم (مفاهيم باليني) است و شامل دو بخش كلي است: 1) محتويات هسته (مفاهيم، توصيفات و ارتباطات) كه همواره ثابت است، 2) محتويات خارج از هسته (نقشه بندي به سيستم هاي طبقه بندي و نامگذاري به اضافه ضمايم محلي) كه برحسب نيازهاي بهداشتي، مديريتي و اصطلاحات بومي هر كشور متفاوت است. چنين ساختاري در كاربردهاي مختلف نرم افزاري مثل پرونده الكترونيك سلامت قابل اجرا است. بحث و نتيجه گيري: ساختار SNOMED CT در كشورهاي آمريكا و انگلستان مورد مقايسه قرار گرفته و با در نظر گرفتن تشابهات (محتويات هسته SNOMED CT) و تفاوتها (محتويات خارج از هسته: نقشه هاي عرضي و ضمايم محلي)، ساختار كلي براي آن در ايران ارايه گرديده است. از آنجايي كه SNOMED CT در كشور استراليا تحت ارزيابي است، پژوهشگر روند تكامل و اجراي آن رادر اين كشور بررسي نموده و با كسب تجربه اجرايي كشورهاي آمريكا و انگلستان در اين زمينه ، راهكارها و چارچوب ساختاري مناسب براي آن در ايران را ارايه نموده است.

Background and Aim: Systematized Nomenclature of Medicine systems are the important supportive for electronic health record in registration and retrieval of data. Systematized Nomenclature of Medicine – Clinical Terms (SNOMED CT) is the most comprehensive language and then the consistency of exchanged data across health care providers and finally the high effectiveness of health care. Materials and Methods : This research is a descriptive – comparative study and has been done in certain time section. Ideally , it have been used the descriptive – comparative method for studying of selective countries. Results : Findings of this study show that American and England countries have accepted the SNOMED CT with application in all clinical activities and Australia country is evaluating this system and is using older versions of SNOMED only in pathologic applications now. Study shows that SNOMED CT structure is complex and based of concept (clinical concept) and contains two fundamental parts : 1) content of SNOMED CT core (concepts , descriptions and relations) that are always constant; 2) content of out of SNOMED CT core (mappings or relation with systems and local extensions) that according to health and management needs and native terms of country are different. Such a structure executes in different software applications. Conclusion : SNOMED CT structure in America and England countries has been compared and with attention to similarities (content of SNOMED CT core) and differences (content of out of SNOMED CT core : cross mappings and local extensions) , a basic SNOMED CT structure has been provided for Iran. Because of SNOMED CT in Australia is under development, researcher has studied its evolution and execution process in this country and has provided an appropriate structural pattern for Iran.
