Ameloblastic fibroma with destruction of the cortex: A case report

مقدمه: آملوبلاستيك فيبروما يك تومور ادنتوژنيك خوش خيم با منشا اپي تليالي و مزانشيمي مي‌باشد كه غالباً در خلف منديبل و معمولاً در دو دهه اول زندگي ايجاد مي‌شود و در ارتباط با دندان نهفته است، بنابراين مي‌تواند باعث تاخير در رويش دندان و يا تغيير در ترتيب رويش دندان گردد. معرفي مورد: در اين گزارش موردي از آملوبلاستيك فيبروما در سمت چپ منديبل در يك خانم 22 ساله كه به علت رفتار تهاجمي و تخريب كورتكس، توسط جراحي تهاجمي تحت درمان قرار گرفته، شرح داده شده است. نتيجه‌‌گيري: در اغلب موارد براي آملوبلاستيك فيبروما درمان محافظه‌كارانه پيشنهاد مي‌شود اما در اين مورد اگرچه در نماي ميكروسكوپي تغييرات بدخيمي مشاهده نشد اما در نماي راديوگرافي، به علت تخريب كورتكس باكال رفتاري تهاجمي از خود نشان داد، از اين‌رو درمان قاطعانه جهت پيش‌گيري از تهاجم و عود انجام گرفت. كليد واژه‌ها: فيلم راديوگرافي، تومورهاي ادنتوژنيك، سي‌تي‌اسكن.

Introduction: Ameloblastic fibroma (AF) is a benign odontogenic tumor with mixed epithelial and mesenchymal origin; it most frequently occurs in the posterior region of the mandible, usually during the first two decades of life. AF is associated with an impacted tooth. Therefore, it might retard tooth eruption or alter the tooth eruption sequence. Case Report: In this report a case of AF on the left side of mandible in a 22-year-old woman is presented; the case was treated with radical excision because of invasive behavior and destruction of the cortex. Discussion: Conservative treatment is suggested for AF most frequently. However, in this case although malignant changes were not detected in microscopic views, radiographic views showed invasive behavior because of destruction of the buccal cortex. Therefore, radical treatment was carried out to prevent invasion and reduce the chance of recurrence. Key words: Computed tomography, Odontogenic tumors, Radiographic film.
