Determinants of Physicians Visits throughout the Different Provinces of Iran: A Case Study of the Population Covered by Social Security Organization

مقدمه: بررسي اثر عوامل مهم در ميزان بهره‌مندي از خدمات پزشكان ابزاري براي سياستگذاران و مديران عرصه سلامت خواهد بود تا ضمن تشخيص جايگاه و اهميت هر كدام از اين عوامل، از نتايج آن در برنامه‌ريزي‌ها و سياستگذاري‌ها استفاده شود. هدف: تعيين ميزان اثر دسترسي، درآمد و پرداخت از جيب بيمار براي ويزيت و دارو، در ميزان بهره‌مندي از ويزيت پزشكان متخصص در ميان افراد تحت پوشش سازمان تامين اجتماعي. مواد و روش‌ها: براي اندازه‌‌گيري تاثير متغيرهاي مورد نظر بر ميزان بهره‌مندي از ويزيت پزشكان متخصص در جامعه 30 ميليون نفري افراد تحت پوشش تامين اجتماعي، از بانك‌هاي اطلاعاتي مديريت‌هاي درمان تامين اجتماعي طي سال‌هاي 88-1377استفاده شد. داده‌هاي جمع‌آوري پس از تعديل، به متغيرهاي مورد نياز در مطالعه تبديل شدند. سپس اين متغيرها به تفكيك استان‌ها در قالب داده‌هاي پنل تنظيم شده و از طريق مدل رگرسيوني دو طرف لگاريتمي با لحاظ اثرات ثابت به محك كشيده شدند. نتايج: يافته‌ها نشان دادندكه افزايش 1%در ميزان دسترسي جغرافيايي و هزينه‌هاي دارويي به ترتيب موجب افزايش 23/0% و17/0% در ميزان بهره‌‌مندي از ويزيت پزشكان متخصص در بخش خصوصي شده‌است. همچنين پرداخت از جيب بيمار بابت ويزيت پزشك متخصص و درآمد بيمه شدگان تاثيري بر ميزان بهره‌مندي از اين خدمت نداشته‌است. نتيجه‌‌گيري: بهره‌‌مندي از ويزيت پزشكان متخصص در بخش خصوصي، نسبت به ميزان پرداخت از جيب بيماران بي‌كشش بوده و حساسيت استفاده از اين خدمت بيشتر متاثر از ميزان دسترسي جغرافيايي به اين خدمت مي‌باشد. سازمان تامين اجتماعي با توجه به ميزان حساسيت استان‌هاي مختلف به مولفه‌‌هاي مختلف مي تواند نسبت به تنظيم و كنترل ميزان بهره مندي بيمه شدگان از اين خدمت اقدام نمايد

Introduction: Understanding the effect of different factors on the utilization of physician services is of great importance for policy making processes in health sector. Identification of such factors will improve the process of planning and policy making in health sector. Objective: To examine the effect of accessibility, income and out- of- pocket payments for visit and medicine on the utilization of specialist doctorsʹ services (visits), in the population covered by Social Security Organization (SSO). Materials and Methods: Managerial data bases throughout the SSO were used to measure the effect of assumed variables on the utilization of specialist doctorsʹ services (visits) in a thirty-million population covered by SSO. The data were collected from 1998 to 2009 and then adjusted for the purpose of the study. A bilogarithmic regression model with panel data, using fixed effect model was estimated. Results: The study results showed that 1% increase in the geographic accessibility and medicine costs would increase the utilization of specialist doctorsʹ services by 23% and 17%, respectively. As well, out-of-pocket payments for specialistsʹ visits and the insuredsʹ incomes have no significant effect on the utilization of these services. Conclusion: Visiting specialists in private sector showed no responsiveness to out-of- pocket payments, but more for geographic accessibility. Social Security Organization can regulate and control the utilization of these services through considering the flexibility of different provinces to different factors.
