The Nursesʹʹ Experiences of Breaking Bad News to the Patients and Their Relatives

مقدمه: كادر پزشكي، بيماران و همراهان بيماران هميشه در معرض مواجهه با اخبار، شرايط و اوضاعي هستند كه آن‌ها را «خبر بد» و «شرايط ناخوشايند» ارزيابي مي‌كنند. هدف از اين پژوهش، تبيين تجربيات پرستاران از رساندن خبر بد به بيماران و همراهان آنان بوده است. روش: در اين پژوهش كيفي با 30 پرستار مصاحبه نيمه ساختار به عمل آمد. نمونه‌گيري مبتني بر هدف و با حداكثر از تنوع انتخاب شدند. حداقل يك سوال باز در تمامي مصاحبه‌ها مطرح شد، بقيه سوالات كاوشي بود. مصاحبه‌ها خط به خط، دست نويس و مطابق رويكرد «تحليل محتوا» آناليز شدند. براي شركت در مطالعه و ضبط صدا رضايتنامه شفاهي اخذ گرديد. يافته‌ها: از آناليز داده‌ها مضامين «به پزشك واگذاركردن، توسل به اقوام دورتر، مهياكردن زمينه، كوچك‌جلوه‌دادن موضوع» استخراج شد. مشاركت‌كنندگان ابلاغ «اخبار تشخيصي» را جز مسيوليت و وظايف خود نمي‌دانستند، لذا تلاش مي‌كردند تا هر آنچه كه در اين حيطه باشد را به پزشك واگذار نمايند. يكي از رايج‌ترين استراتژي‌هاي پرستاران در رساندن خبر بد «روحيه دادن به بيمار و همراه وي» بوده است. نتيجه‌گيري: داده‌ها نشان داد پرستاران در ابلاغ خبر بد به بيماران و همراهان وي از روش‌هاي غير مستقيم استفاده مي‌كنند، پزشكان را مسوول اين كار مي‌دانند، دستورالعمل خاصي در خصوص ابلاغ اخبار بد به بيمار و خانواده ندارند و هر پرستاري به سبك خاص خود مبتني بر ذهنيات، ولي مطابق فرهنگ رفتار مي‌نمايد. با توجه به تنوع اجتماعي و فرهنگي بيماران، ضروري است تا پرستاران با شيوه‌هاي علمي ابلاغ خبر بد كه به كرات در زندگي كاري خود با آن مواجهه مي شوند، آشنا شوند.

Introduction: The health care providers, patients and their relatives often face to the conditions and unpleasant circumstances such as bad news. Our purpose of this study was to explore the nursesʹ experiences of breaking bad news to the patients and their relatives. Method: In this qualitative research, semi-structured interviews conducted with 30 nurses. The sampling method was purposeful and with maximum variation. All of the participants answered at least to one open-ended question "have you ever told a bad news to your patients or their relatives? If so, explain your experiences" The probing questions used to make deeper interviews. All of the interviews were recorded, transcribed verbatim and analyzed using "conventional content analysis" method. The oral informed consent was obtained for the participation in the study and voice recording. Results: our analysis showed that "avoiding and delegating to the physician, seeking help from distant relatives, providing a context, minimizing the importance of situation" are main themes for the nurse’s experiences with breaking bad news. The participants believed that giving diagnostic bad news is not their responsibility, hence, tried to delegate giving bad news to the physicians. One of the most common strategies for giving bad news was "give hope to the patients and their attendants" Conclusion: The nurses used the uncertain procedures to give bad news to the patients and their relatives, because as their idea, physicians are responsible. So, since there was no protocol or guideline for breaking bad news to apply in practice, the participants used their own personal and cultural based styles. With consideration of social and cultural differences among the patients, and high frequency of this working experience for the nurses, learning the ethical ways of giving bad news seems very essential.
