بررسي همبستگي مقدار wit’s با روشي جديد در ارزيابي رابطه قدامي – خلفي فكين

Evaluation of correlation between Witʹʹs appraisal and a new method for assessment of sagittal relationship of jaws

چكيده زمينه و هدف: با وجود ارزشمند بودن مقدار wit’s در بيماران ارتودنسي براي تعيين رابطه قدامي خلفي فكين، در برخي از بيماران به دلايل متعدد رسم پلن اكلوزال دشوار است و يا شيب آن بسيار تغيير كرده است. در اين مطالعه روشي جديد براي ارزيابي روابط قدامي خلفي فكين معرفي مي‌شود. بنابراين هدف از اين مطالعه بررسي همبستگي بين اين روش و مقدار witʹs مي‌باشد. روش بررسي: اين مطالعه بر روي سفالومتري لترال 71 بيمار ارتودنسي از هر سه گروه مال اكلوژن اسكلتي (ClI ، ClII و ClIII) انجام شد. عمودي از نقطه A وB بر پلن اكلوزال فانكشنال (اندازه‌گيري wit’s ) و همچنين پلن فرانكفورت رسم شده و فاصله ميلي‌متري بين آنها در هر دو مورد اندازه‌گيري گرديد. توافق بين wit’s و فاصله AOʹ و BOʹ بر پلن فرانكفورت (q) به وسيله Pearson Correlation test محاسبه شد. روابط همبستگي هر يك از اين مقادير با SNA ، SNB ، ANB و Overjet نيز ارزيابي گرديد. يافته‌ها: رابطه همبستگي بالايي بين اندازه‌گيري با استفاده از پلن فرانكفورت و wit’s به دست آمد، (82/0=r ). هر دو روش ارتباط آماري معني‌داري را با SNA ، SNB ، ANB و Overjet نشان دادند (05/0 < p) و بالاترين رابطه همبستگي با زاويه ANB را داشتند. نتيجه‌گيري: استفاده از اندازه‌گيري q به اندازه استفاده از روش معمول محاسبه wit’s ارزشمند بوده و در مواردي كه تعيين پلن اكلوزال با مشكل مواجه است، اين روش مي‌تواند جايگزين مناسبي براي اندازه‌گيري wit’s باشد. كليد واژه‎ها: اندازه گيري wit’s – پلن فرانكفورت – پلن اكلوزال فانكشنال

Abstract Background and Aim: Although Wit’s appraisal is a valuable measurement for antero-posterior jaw relationship evaluation in orthodontic patients, occlusal plane detection is difficult in a lot of cases or is challenging due to its various inclinations. In this study a new method for assessment of sagittal skeletal relationship is introduced and the purpose of this investigation is evaluation of correlation between wit’s appraisal and this new method. Materials and Methods: Lateral cephalograms of 71 orthodontic patients (male and female) from all types of skeletal malocclusions (class I, II, III) were collected and analyzed. Two lines were traced from point A and point B while they were perpendicular to Frankfort Horizontal plane (FH) and also to functional occlusal plane (Witʹs) and the bothdistances were measured. Pearson Correlation analysis was performed for assessment the relationship between AO´-BO´ distance on FH (q) and wit’s appraisal. Correlation of these variables was evaluated with SNA, SNB, ANB and overjet, separately. Results: The results showed a high correlation of variables using Frankfort Horizontal plane and wit’s appraisal (r= 0.82). Both methods revealed significant statistical relation with SNA, SNB, ANB and overjet (p < 0.05). The highest scores of correlation with ANB were obtained in using both methods. Conclusion: The use of measurement is as much valuable as the usual wit’s measurement and it can be a good alternative in patients when tracing the accurate occlusal plane is impossible or difficult.
