بررسي ميزان كارايي پروتزهاي اندام فوقاني در انجام فعاليت‌هاي مختلف در افراد آمپوته اندام فوقاني

Evaluation of the performance of the prostheses during daily activities in upper limb amputees

چكيده مقدمه: افراد قطع عضو اندام فوقاني با كاهش چشمگير توانايي در انجام فعاليت‌هاي خود رو به رو هستند. براي جبران اين امر از انواع مختلف پروتز استفاده مي‌شود، كه مطابق با مكانيزم حركتي ممكن است كازمتيك، مكانيكال و يا مايو‌الكتريك باشند. متاسفانه بر خلاف تلاش‌هاي فراوان در زمينه افزايش كارايي پروتزهاي اندام فوقاني، ميزان موفقيت افراد در به كار بردن پروتز، پايين مي‌باشد‍‍. از اين رو در اين تحقيق سعي بر آن است تا ميزان كارايي پروتزها با توجه به انواع فعاليت مورد بررسي قرار گرفته، ارتباط آن با نوع پروتز، سمت قطع عضو و سطح قطع عضو تعيين گردد. مواد و روش‌ها: تعداد 76 نفر از آمپوته‌هاي اندام فوقاني در استان‌هاي تهران، اصفهان و شيراز كه به مراكز ارتوپدي فني مراجعه كرده بودند، مورد بررسي قرار گرفتند. فاكتورهايي همچون ميزان توانايي افراد در انجام فعاليت‌هاي مختلف، سن فرد، طرف قطع عضو، سطح قطع عضو انتخاب گرديد. يافته‌ها: نتايج اين تحقيق نشان داد كه بر خلاف سيستم حركتي به كار گرفته در پروتز، اكثر افراد از پروتز خود فقط در حد يك تكيه گاه استفاده مي‌كنند. ميزان توانايي آمپوته‌هاي اندام فوقاني در انجام فعاليت‌هاي مختلف همانند رانندگي كردن و نيازهاي شخصي از فعاليت‌هاي ديگر بيشتر بود. گر چه انتظار مي‌رفت كه پروتزهاي مكانيكال نيازهاي فرد را بهتر برآورده سازند، با اين حال در خيلي از سطوح قطع عضوي تفاوت ملموسي مشاهده نشد (1/0 = (P. نتيجه‎گيري: از ميان فاكتورهاي مختلفي كه مي‌تواند بر ميزان كاركرد پروتزهاي اندام فوقاني اثر‌‌گذار باشد، تاثير آموزش در استفاده از پروتز از اهميت به سزايي برخوردار است. پيشنهاد مي‌شود كه از تكنيك‌هاي جديد و مواد مناسب‌‌تر جهت بالا بردن كارايي پروتز‌ها در اين نوع از آمپوته‌ها استفاده گردد. كليد واژه‌ها: پروتز اندام فوقاني، كارايي، سمت قطع عضو.

Abstract Introduction: Upper limb amputees have a lot of problems in undertaking daily activities. In order to increase the performance of the upper limb amputees various kinds of prostheses have been developed which can be divided into mechanical and cosmesis ones depends upon their motion mechanism employed. The abilities of the clients to use their prostheses are not as good as expected. So the main aim of this research project was to find the amounts of ability of the subjects to use their prosthesis during undertaking daily activities. Moreover, it was planned to find the influence of type of prosthesis, side of amputation and level of amputation on the ability of the subjects to use their prostheses. Materials and Methods: Seventy six patients with upper limb amputations were recruited in this research project. Some parameters such as the ability of the subjects to use the prostheses, age of participants, side and level of amputation were selected for final analysis. Results: The results of this research project showed that although most of the patients received mechanical prostheses, they use their artificial limbs only as a supportive lever. However their abilities to use their prostheses while driving or doing personal hygiene was more than that of other activities. There was no difference between the performance of the subjects with mechanical and cosmesis prostheses in this research project. Conclusion: Amongst various parameters which influence the performance of the upper limb prostheses, the influence of training is more important. It is recommended to manufacture the upper limb prostheses from new materials and by use of the new methods. Keywords: Upper limb prosthesis, efficiency, side of amputation.
