بررسي اثر ناتواني بر ميزان مواجهه با موانع محيطي مشاركت در كودكان فلج مغزي

Influence of childʹʹs disability on encountering environmental barriers to participation of children with cerebral palsy

چكيده مقدمه: فلج مغزي شامل ناهنجاري‌هاي غير پيش‌رونده به مغز در حال رشد است كه منجر به بروز دسته‌اي از مشكلات حركتي مي‌شود. علاوه بر مشكلات حركتي كه بر ميزان مشاركت كودك فلج مغزي تاثيرگذار است، عوامل محيطي نيز مي‌تواند ميزان مشاركت را كاهش يا افزايش دهد. هدف اين مطالعه، بررسي اثر ناتواني بر ميزان مواجهه با موانع محيطي بود. مواد و روش‌ها: 75 نفر از والدين كودكان فلج مغزي 12-5 ساله در دو مركز توان‌بخشي تهران به صورت در دسترس به تكميل پرسش‌نامه عوامل محيطي بيمارستان Craig (Craig hospital inventory of environmental factors يا CHIEF) و پرسش‌نامه ويژگي‌هاي فردي پرداختند. محقق با گرفتن اطلاعات از والدين به تعيين سطح توانايي دستي، عملكرد حركتي و سطح شناختي كودك پرداخت. يافته‌ها: والدين كودكان فلج مغزي بيشترين ميزان مواجهه اين كودكان را با موانع به ترتيب در خرده مقياس سرويس‌ها، كمك و سياست‌ها گزارش نمودند. مقايسه نمره خرده مقياس‌ها و نمره كل از نظر سطح شناختي، توانايي دستي و عملكرد حركتي درشت كودك تفاوت معني‌داري را نشان داد. نتيجه‎گيري: موانع محيطي متعددي براي مشاركت كودكان فلج مغزي وجود دارد و با كاهش عملكرد كودك ميزان مواجهه با اين موانع بيشتر خواهد بود. اصلاح موانع محيطي موجود و فراهم نمودن تسهيلات متناسب با معلوليت كودكان براي مشاركت آن‌ها ضروري است. كليد واژه‌ها: موانع محيطي، فلج مغزي، مشاركت، كودك و ناتواني

Abstract Introduction: Cerebral palsy (CP) is included a group of non-progressive disturbances that occur in developing brain and led to permanent movement disorders. Although childrenʹs participation is influenced by their movement disorders, environmental factors can be considered as barriers or facilitators to their participation. This study was aimed to investigate the influence of childʹs disability on encountering environmental barriers to participation of children with cerebral palsy. Materials and Methods: Seventy five parents of children with CP were participated in this study. They aged 5-12 years to include for this research. The Craig Hospital Inventory of Environmental Factors (CHIEF) and personal characteristic questionnaires were used to collect the data. Also childrenʹs gross motor, manual ability and cognitive level were asked from parents by researcher. Results: Barriers to participation most commonly reported by parents found in services and assistance and policies subscales. Cognitive, gross motor functioning and manual ability levels were related to the parent perceived barriers. Conclusion: Findings from this study indicated that the presence of multiple environmental barriers to participate these children in activities. With low function all children encountered more barriers. We can enhance participation by minimizing these barriers and providing more facilitators.
