بررسي دقت راديوگرافي پانوراميك در تعيين فاصله سوراخ منديبل نسبت به پلن اكلوزال و لبه قدامي راموس

Accuracy of panoramic radiographs in determining the distance between mandibular foramen and occlusal plane and anterior border of ramus

چكيده مقدمه: عصب دنداني تحتاني، مسوول عصب‌دهي حسي دندان‌ها و استخوان فك پايين است. داشتن اطلاعات دقيق از محل سوراخ منديبل بسيار مهم است. اين محل علاوه بر داشتن اهميت در ايجاد بي‌حسي در عصب مذكور، در بسياري از جراحي‌هاي دهان و فك در معرض آسيب است. هدف مطالعه حاضر، بررسي دقت راديوگرافي پانوراميك در تعيين موقعيت سوراخ منديبل بود. مواد و روش‌ها: در اين مطالعه مقطعي و بدون جهت، 50 راديوگرافي پانوراميك ولترال سفالومتري از افراد بين سنين 30-25 ساله مراجعه كننده به كلينيك افضل شهر اصفهان در بهار سال 1391 جمع‌آوري گرديد. با استفاده از كوليس فاصله مركز سوراخ منديبل تا پلن اكلوزال و لبه قدامي راموس تعيين شد. داده‌ها توسط آناليزهاي آماري t و ضريب پايايي در نرم‌افزار SPSS نسخه 5/11 تجزيه و تحليل شد (05/0 = ?). يافته‌ها: آزمون t نشان داد كه ميانگين فاصله سوراخ منديبل تا پلن اكلوزال (004/0 = p value) و فاصله آن تا لبه قدامي راموس (001/0 > p value) در راديوگرافي پانوراميك نسبت به مقادير واقعي به دست آمده از راديوگرافي لترال سفالومتري به طور معني‌داري بيشتر است. بين فاصله سوراخ منديبل تا پلن اكلوزال در راديوگرافي پانوراميك و مقادير واقعي به دست آمده از راديوگرافي لترال سفالومتري مشابهت وجود داشت (0001/0 > p value، 8315/0 = r). همچنين بين فاصله سوراخ منديبل تا لبه قدامي راموس در راديوگرافي پانوراميك و مقادير واقعي به دست آمده از راديوگرافي لترال سفالومتري مشابهت وجود داشت (0001/0 > p value، 9126/0 = r). نتيجه‌‌گيري: با توجه به محدوديت‌هاي اين مطالعه مي‌توان بيان نمود كه راديوگرافي پانوراميك در هر دو بعد افقي و عمودي نتايج مشابهي با تكنيك استاندارد لترال سفالومتري دارد و با رعايت اصول مي‌توان براي تعيين محل سوراخ منديبل از آن استفاده كرد. كليد واژه‌ها: منديبل، راديوگرافي پانوراميك، عصب دنداني تحتاني

Abstract Introduction: Inferior alveolar nerve is responsible for the sensory innervation of the mandible and its teeth. Knowledge about the exact location of mandibular foramen is very important due to its important role in the success of mandibular block injection and possibility of nerve injuries during oral and maxillofacial surgeries. The aim of this study was to evaluate the accuracy of panoramic radiographs in determining the location of mandibular foramen. Materials and Methods: In this random cross-sectional study 50 panoramic and lateral cephalometric radiograms were collected from patients, 20?50 years of age, referring to Afzal Dental Clinic in Esfahan in spring 2012. Measurements were taken by a caliper from the center of the foramen to the occlusal plane and anterior border of ramus. Data was analyzed by t-test and intraclass correlation coefficient (ICC) using SPSS 11.5 (? = 0.05). Results: According to t-test, mean distances from the center of the foramen to the occlusal plane (p value = 0.04) and to the anterior border of ramus (p value < 0.001) on panoramic radiographs were significantly greater than the actual distances measured on lateral cephalograms. In addition, there were similarities between the distances from the mandibular foramen to the occlusal plane (p value < 0.001, r = 0.8315) and the anterior border of ramus (p value < 0.001, r = 0.9126) on panoramic radiographs and the actual distances measured on lateral cephalograms. Conclusion: Under the limitations of the present study, it can be concluded that measurements on panoramic radiographs are similar to those on lateral cephalograms in both anteroposterior and vertical dimensions and on the condition that radiographic principles are observed they can be used to determine the location of mandibular foramen.
