Medical Professionalism and its Education to Medical Students

لغت لاتين «Professio» به معناي «اعلان عمومي و تعهد دادن به جامعه» است و واژه پروفشنال به طور سنتي براي سه رشته «پزشكي»، «وكالت» و «علوم ديني» استفاده شده است. حرفه شريف پزشكي كه دراصل نوعي «فراخوان» از طرف افرادي با صلاحيت و دانشمند بود كه نيازهاي بيماران را بر خواسته هاي خود ترجيح مي دادند، به نظر مي رسد كه در حال تبديل به يك تجارت «پول بده و سرويس بگير» مي باشد و اين مساله نه تنها در جامعه ما، بلكه در بسياري از جوامع پيشرفته و در حال پيشرفت به صورت كاهش اعتماد جامعه به پزشكان نمود پيدا مي كند. انتظارات خانواده، كاهش اساتيد فرزانه و الگو (Role Model) براي دانشجويان، پيشرفت هاي تكنولوژيك، تخصص گرايي و فوق تخصص گرايي، و درگير شدن در سيستم خدمات بهداشتي درماني به صورت دست دوم و دست سوم [سيستم ارجاعي] به عنوان علل اين مساله درنظر گرفته شده اند. در واقع جامعه پزشكي به آهستگي در حال نوعي «تجزيه شدن» است. از مدت ها قبل در دانشكده هاي پزشكي كشورهاي غربي، به ضرورت آموختن و آموزش دادن اصول حرفه اي گري به دانشجويان پزشكي تاكيد شده و امروزه به ارتقاي كيفيت و كميت اين آموزش ها بسيار پرداخته مي شود تا ذهنيت هاي درست، مهارت هاي مناسب و شايسته و يك سيستم «ارزشي» در مجموعه آموزش هاي پزشكي ايجاد شود. بايستي دوره تحصيلات پزشكي به گونه اي مورد بازبيني و مداقه قرار گيرد كه موضوع مهم «آموزش حرفه اي گري پزشكي» در تمامي سطوح آموزش وارد گردد. در اين نوشتار بر آنيم كه ضمن معرفي مختصر مفهوم پروفشناليزم يا حرفه اي گري پزشكي و مباني آن، اندكي درباره شيوه هاي ياددهي و يادگيري آن و موانع موجود تامل نماييم.

The Latin word of »profession« means public declaration with the force of a promise for a general benefit, and »professionalism« traditionally has been applied for »clergy«, »law« and »medicine«. The noble profession of medicine, taken up as a “calling” by those who are expected to put the needs of the patient above their own, appears to have become a "fees-for-service" business model and trade, and this reflect itself at the societies as the reduction in thrust to the physicians. Parental expectations, lack of role models, technological advancements, specialization and sub-specialization, and second and third-party involvement in the healthcare delivery system [referral system] have been identified as reasons for these concerns. Indeed the medical society is being degraded gradually. From many years ago, many medical communities in western countries have emphasized the need for learning and teaching the professionalism in medical schools and enhancing its quality and quantity , in order to development of good attitudes, appropriate and competent skills, and establishment of a "value system" in medical education system that reflects the tenets of professionalism in medicine. The medical curriculum will need to be scrutinized so as to introduce the important subject of professionalism at all levels of training and education. In this article we are trying to define medical professionalism and its principles, and also briefly refer how to learn and teach medical professionalism to medical students and the barriers to that.
