The Facilitators of the Transition to Heamodialysis

زمينه و هدف: گذار عبور از يك مرحله زندگي، شرايط يا وضعيت به ديگري است. بيشتر مراقبت هاي پرستاري طي فرايند گذار روي داده و هدف مراقبت پرستاري رسيدن به نتايج مطلوب براي بيمار و رسالت آن در قالب اين مفهوم، علم و هنر تسهيل گذار به سلامتي و رفاه انسان است. اين در حالي است كه بيماران مراحل انتهايي كليوي ناگزير به گذار از اين وضعيت به وضعيت جديد همودياليز هستند. هدف اين مقاله تبيين عوامل تسهيل كننده در فرايند گذار به همودياليز مي باشد. روش بررسي: اين مطالعه بخشي از يك مطالعه كيفي (نظريه مبنايي) در رساله دكترا مي باشد. نمونه پژوهش شامل بيماراني كه در يكسال اخير تحت همودياليز قرار داشتند و ساير مطلعين مرتبط با آنها بود. نمونه‌گيري بصورت هدفمند و سپس نظري صورت گرفته و تا اشباع داده ها ادامه يافت. محيط پژوهش بخش همودياليز بيمارستان هاي وابسته به دانشگاه علوم پزشكي شهيد بهشتي شهر تهران بود. جمع آوري داده ها با مصاحبه هاي نيمه ساختار يافته، ثبت هاي يادآور و يادداشت هاي عرصه انجام و با 24 نفر به اشباع رسيد. تحليل داده ها به روش مقايسه مداوم صورت گرفت. دقت مطالعه با روش لينكلن و گوبا تاييد شد. يافته ها: عوامل تسهيل كننده در فرايند گذار به همودياليز عوامل فردي (ويژگي هاي جسمي، رواني، اجتماعي و معنوي) و عوامل اجتماعي (حمايت تخصصي تيم پزشكي و پرستاري، تشريك مساعي خانواده و آشنايان، پشتيباني سازمان هاي اجتماعي، اثرات تعامل با همتايان) بودند. نتيجه گيري كلي: توجه ويژه پرستاران به عوامل فردي و اجتماعي، فراهم آوردن بستري مناسب براي شناسايي و تقويت آنها و انجام اقدامات حمايتي مناسب نقش بسزايي در فرايند گذار سالم به همودياليز خواهند داشت.

Background &Aims: Transition is a passage or movement from one state, condition, or place to another. Most of nursing care occurs during transition process and the primary goal of nursing is to enhance healthy outcomes for patients. The mission of nursing according to this concept is the art and science of facilitating the transition to health and well-being. In the meanwhile, end stage renal disease patients have to transit from this situation to the hemodialysis situation. The purpose of this study is to explore facilitators in the process of transition to heamodialysis. Material & Methods: This paper is a part of findings of a more extensive grounded theory study. The participants included new hemodialysis patients, their relatives and health care personnel. Purposeful and theoretical sampling was used to recruit participants and continued until saturation. The study was undertaken in hospitals affiliated to Shaheed Beheshti and Tehran University of Medical Sciences in Tehran. Data was generated by semi-structured interviews, and observation. The data were analyzed by constant comparison. The rigor of the study was confirmed through the criteria proposed by Lincoln and Guba. Results: Facilitators in the process of transition to heamodialysis included personal factors (physical, mental and spiritual properties), social factors (expert support of medical and nursing team, collaboration of family and familiars, support of social organizations and effects of interaction with peers) and spirituality (relating with God, having recourse to Godʹs Saints and relying on spirituality values). Conclusion: Nurses’ special attention to the personal, social and spiritual characteristics of hemodialysis patients, providing an appropriate context for identifying and reinforcing these factors, and doing appropriate supportive interventions would play an important role in the process of healthy transition to heamodialysis.
