بررسي اثرات اسيد دوكوزاهگزانوييك بر ريشه‌كني عفونت هليكوباكترپيلوري، برخي فاكتورهاي التهابي و ظرفيت تام آنتي اكسيداني سرم

The effects of Docosahexaenoic Acid (DHA) on Helicobacter pylori eradication, some serum inflammatory factors and total antioxidant capacity

زمينه و هدف: با توجه به شيوع بالای عفونت هليكوباكترپيلوری و عوارض ناشی از مزمن شدن آن، هم‌چنين فعاليت ضدباكتريايی عليه هليكوباكترپيلوری و خواص ضدالتهابی اسيدهای چرب امگا-3، مطالعه حاضر با هدف بررسی اثر مكمل ياری با اسيد دوكوزاهگزانوييك (DHA) Docosahexaenoic Acid بر ريشه‌كنی عفونت هليكوباكترپيلوری و سطح برخی نشانگرهای التهابی و ظرفيت تام آنتی‌اكسيدانی سرم انجام گرفت. روش بررسی: در يك كارآزمايی بالينيِ دوسوكور تصادفيِ كنترل شده، 66 بيمار آلوده به هليكوباكترپيلوری (33 مورد و 33 شاهد)، در كنار درمان چهار دارويی عفونت، به‌طور تصادفی روزانه دو گرم مكمل morDHA يا روغن MCT به عنوان دارونما، به مدت 12 هفته دريافت كردند. دريافت غذايی افراد توسط يادآمد 24 ساعته خوراك جمع‌آوری و با نرم‌افزار Nutritionist IV آناليز شد. نمونه‌گيری از خون ناشتا، اندازه‌گيری وزن، قد، نمايه توده بدنی (BMI) و سطح فعاليت بدنی در ابتدا و انتهای مطالعه انجام گرفت.     يافته‌ها: ميزان ريشه‌كنی عفونت، سطح اينترلوكين-6 (IL-6)، پروتيين واكنش‌گر C با حساسيت بالا (hs-CRP) و ظرفيت تام آنتی‌اكسيدانی (TAC) در انتهای مطالعه، بين دو گروه، تفاوت معنی‌دار نداشت، در حالی كه سطح اينترلوكين-8 (IL-8) بين دو گروه متفاوت بود (008/0P=). اختلاف غلظت بين ابتدا و انتهای مطالعه نيز در هيچ يك از فاكتورها بين دو گروه معنی‌دار نبود. نتيجه‌گيری: دريافت مكمل morDHA در بيماران آلوده به هليكوباكترپيلوری، بر ريشه‌كنی عفونت، سطح سرمی فاكتورهای IL-6، hs-CRP و TAC اثر معنی‌دار نداشت، در حالی كه بر سطح IL-8 مؤثر بود.

Background: As regard to high prevalence of Helicobacter pylori infection and complications of itʹs persistence, as well as anti-bacterial activity against of Helicobacter pylori and anti-inflammatory properties of omega-3 fatty acids, this study was conducted to evaluate the effects of supplementation with Docosahexaenoic Acid (DHA) on the eradication of Helicobacter pylori infection, some serum inflammatory markers and total antioxidant capacity.  Methods: In a double-blind, placebo-controlled clinical trial, 66 H. pylori positive patients (33 in the intervention group and 33 in the control group), along with tetra-drugs H. pylori eradication regimen, randomly received daily two grams morDHA supplement or Medium Chain Triglyceride (MCT) oil as placebo for 12 weeks. Dietary intake data was collected by 24 hour food recall and analyzed by Nutritionist IV software. Sampling from fasting blood and measuring weight, height, body mass index (BMI) and level of physical activity were done at the first and the end of the study. As well as, eradication test of the infection was performed for all patients at the end of the intervention.  Results: Eradication rate of the infection, the level of interleukin-6 (IL-6), high sensitivity C-reactive protein (hs-CRP) and total antioxidant capacity (TAC) didnʹt have significant difference between two groups at the end of the study (P>0.05), while the level of interleukin-8 (IL-8) was different between two groups (P=0.008). Difference of the concentration between the beginning and the end of the study was not significant in any factors between two groups (P>0.05).   Conclusion: Intake of morDHA supplement didnʹt have significant effect on the eradication of H. pylori, serum levels of IL-6, hs-CRP and TAC, while it was effective on the level of IL-8.
