A Review on Regenerative Endodontics

مروري بر درمانهاي رژنراتيو در اندودنتيكس

چكيده زمينه و هدف: درمانهاي رژنراتيو با هدف بازسازي بافتهاي از دست رفته دندان به سرعت در حال پيشرفت است. هدف از اين مقاله، مروري بر درمانهاي رژنراتيو در اندودنتيكس مي‌باشد. روش بررسي: در اين مطالعه مروري مقالات مرتبط با مهندسي بافت و فرآيندهاي رژنراتيو منتشر شده به زبان انگليسي از ماه May سال 2000 تا ماه May سال 2013 در پايگاه PubMed به صورت سيستماتيك با استفاده از كليد واژه‌هاي -Cell and Tissue based therapy، Regeneration ، Stem cell و Tissue engineering جستجو شد و مقالات معتبرتر انتخاب شد. يافته‌ها: جهت بازسازي هر بافتي سه عنصر سلول بنيادي، عامل رشد و داربست مناسب ضروريست. ري واسكولاريزيشن يك درمان رژنراتيو است كه از يك داربست فيبريني حاصل از لخته خون و عوامل رشد نهفته در ديواره‌هاي عاجي و سلول‌هاي بنيادي اطراف ريشه استفاده مي‌كند تا ادامه تكامل ريشه دندان نابالغ با نكروز پالپي را منجر شود. بنابراين، ري واسكولاريزيشن يك درمان واقع گرايانه و با ارزش در موارديست كه پيش آگهي ضعيفي دارند. نتيجه‌گيري: درمانهاي رژنراتيو با هدف نهايي ساخت يك دندان دايمي كاملاً شكل گرفته، آغاز تحولي در علم دندانپزشكي مي‌باشد. البته بازسازي دندان كامل بسيار پيچيده‌تر از درمان دندان نابالغ با اپكس باز است كه داراي منبعي از سلول‌هاي بنيادي مي‌باشد و لذا نيازمند تحقيقهاي بسياري است. كليد واژه‎ها: درمانهاي رژنراتيو ، مهندسي بافت ، سلول‌هاي بنيادي

Abstract Background and Aim: Regenerative endodontics with the goal of replacing the lost den-tal tissues is developing fast. The aim of this study was to review the recent progressions, known limitations and advantages of regenerative treatments compared to other possible treatment modalities. Materials and Methods: We searched MedLine and PubMed systematically for articles published from May 2000 until May 2013 in English which were relevant to tissue engi-neering and regenerative procedures using the keywords “cell and tissue based therapy”, “regeneration”, “stem cell” and “tissue engineering”. More valid papers were chosen. Results: Stem cells, growth factors and an appropriate scaffold are the three essential el-ements required for tissue regeneration. Revascularization is an ideal regenerative treat-ment, which uses a fibrin scaffold. This scaffold is made of peri-radicular stem cells, a clot and latent growth factors in dentinal walls. The clinical outcome of revascularization is complete root formation in a premature necrotic permanent tooth. Thus, revasculariza-tion is a realistic and worthwhile approach in patients with fair to good prognosis. Conclusion: Regenerative treatments with the aim of producing a completely formed permanent tooth are going to revolutionize dental science. Regeneration of a fully formed tooth is much more complicated than treatment of an open apex tooth and this issue re-quires further studies.
