بررسي كيفيت زندگي در بيماران مبتلا به ديابت نوع دو و ارائه يك مدل ساختاري

Determination quality of lifein patients with type II diabetes andpresentation a structural model

سابقه و هدف : يكی از مفاهيم بسيار مهم در ديابت نوع دو به عنوان يك بيماری مزمن كيفيت زندگی است كه بر عملكرد فردی و اجتماعی اثرات گسترده ای دارد . با توجه به عوارض كوتاه مدت، بلند مدت و هزينه های بسيار زياد اين بيماری، مطالعه حاضر با هدف تعيين كيفيت زندگی اين بيماران انجام شد و در نهايت يك مدل ساختاری ارائه گرديد. مواد و روش ها : اين مطالعه به صورت مقطعی بر روی 250 بيمار ديابتی نوع دو در يزد ا نجام شد در اين مطالعه از استفاده شد اين ابزا ر شامل 34 سوال و 8 بعد محدوديت نقش ناشی از سلامت جسمانی، استقامت بدنی، QOLID ابزار سلامت عمومی، رضايت از درمان، علايم رنجوری، نگرانی های مالی، سلامت روانی و رضايت از رژيم غذايی می باشد . تحليل گرديد. Amos ( و ( 21 SPSS ( داده ها با استفاده از نرم افزار( 16 يافته ها : در اين مطالعه 42 درصد از بيماران مرد و 58 درصد زن بودند .بين كيفيت زندگی با سطح تحصيلات، ارتباط معنی داری وجود داشت بر طبق اين مطالعه، حيطه های سلامت عمومی و (p< 0/ درآمد و مصرف سيگار با ( 05 استقامت بدنی كم ترين مقدارميانگين را داشتند. در نتيجه ديابت دراين حيطه ها بيش تر بر كيفيت زندگی بيماران اثر می گذاشت. در مدل ساختاری، اثر مستقيم استقامت بدنی بر سلامت عمومی ، محدوديت نقش، سلامت روان ورضايت از 0) بود. اثر مستقيم سلامت عمومی و محدوديت نقش بر رضايت از /19 ، 0/29 ، -0/ 57 ،0 / درمان به ترتيب برابر با ( 69 0 معن يدار شدند. اثر غيرمستقيم استقامت بدنی / 0- ) شد. همه اثرات مستقيم در سطح 05 / 0و 13 / درمان به ترتيب برابر ( 30 0 معنی دار شد. / 0 شد كه اين اثر در سطح 05 / بر رضايت از درمان برابر با 28 استنتاج: ديابت بيماری مزمنی است كه بركيفيت زندگی بيماران به شدت ت أثير می گذارد . هر گونه تلاش در مدل های ساختاری پيش بينی كننده كيفيت زندگی، نقشی معنی داری در حل دشواری های اين بيماران ايفا می كند.

Background and purpose: One of the important concepts in diabetic mellitus as a chronic disease is quality of life that it has extensive effects on social and individual performance.Due to the short-term and long- term effects and expensive costs of these diseases,the present study was carried out to determine the quality of life of these patients and finally was presented a structural model. Material and Methods: This study was performed on250patients with type IIdiabetes in Yazd.it was used from the questionnaire of standard quality of life(QOLID)as the data collection tool that contains a total of34questions and8domains Role Limitation Due to Physical Health,Physical Endurance,General Healt,Treatment Satisfaction,Symptom Bothe,Financial Worries,Emotional/Mental Health,Diet Satisfaction . Data were analyzed by using from spss(16) and amos(21)softwares. Results: in this study درصد 42 of the patients were male and درصد 58 were female.Betweenquality of lifeandage groupsEducation,income and smoking,there was significant relationship(P<0.05).According to the study, domains general health and physical endurance had the lowest mean.according to the result, diabetes in the domains on quality of life of diseases had the most influence.in structural model,Direct effect of endurance physically on general health,role limitation,mental healthand treatment satisfaction,was(0.69, 0.57, -0.29,0.19)respectively.Direct effect of general health and role limitations on Treatment Satisfaction was(0.30,-0.13) respectively.All of the direct effects were significanted at the level of 0.05.Indirect effect physical endurance on treatment satisfaction was0.28that it was significanted at the level of0.05. Conclusion: Diabetes is a chronic disease that severely effects on the patientʹs quality of life.Every attempt in predicting structural models of quality of life play a meaningful role in the solution of patientʹs problems.
