بررسي ارتباط فاكتور V ليدن و جهش ژن پروترومبين با سكته قلبي حاد

Investigating Factor V Leiden and Prothrombin G20210A Gene Mutation in Relation with Acute Myocardial Infarction

چكيده سابقه و هدف: سكته قلبی در افراد جوان پديده شايعی نيست، اما به علت ميزان بالای مرگ و مير ناشی از آن، مسائل روحی و روانی و اقتصادی زيادی همراه دارد. در اين مطالعه ارتباط ميان فاكتورهای ژنتيكی و غير ژنتيكی با سكته قلبي حاد در افراد كم تر از 50 سال مورد بررسی قرار گرفته است. مواد و روش ها: مطالعه ی حاضراز نوع مورد- شاهدی بود كه روی 101 بيمار كم تر از 50 سال با تشخيص سكته قلبي مراجعه كننده به بيمارستان حضرت فاطمه الزهرا (س) ساری و 101 فرد به عنوان شاهد انجام گرفت. اطلاعات دموگرافيك RFLP-PCR ليدن و پروترومبين هر يك از بيماران و گروه شاهد با روش V و عوامل خطر ثبت شد و ژن های فاكتور مورد بررسی مولكولی قرار گرفت. 4 درصد ) و در يك / 94 درصد) نرمال، در 10 مورد هتروزيگوت ( 95 / ليدن در 191 نفر ( 55 -V يافته ها: ژن فاكتور 6 درصد ) و در گروه شاهد 3 بيمار / 0 درصد) گزارش شده بود . در گروه مورد 7 بيمار ( 9 / مورد نيز هوموزيگوت ( 49 نيز در 197 نفر G20210A جهش ژن پروترومبين .(p=0/ ليدن هتروزيگوت بودند ( 27 -V 2/97 درصد) از نظر فاكتور ) 2 درصد) هتروزيگوت گزارش شده بود كه بين دو گروه / 97/5 درصد) از افراد مورد بررسی نرمال و تنها در 5 نفر ( 5 ) .(p=0/ اختلاف آماری معناداری مشاهده نشد ( 17 در اين افراد نسبت به افراد نرمال (G20210A ليدن و پروترومبين V استنتاج: شيوع دو فاكتور ژنتيكی ( فاكتور اختلاف معنی داری نداشت. مطالعات بيشتر جهت لزوم بررسی اين فاكتورها در اين گروه سنی توصيه می گردد.

Background and purpose: Beside the environmental determinants there are major genetic factors that could cause Myocardial Infarction (MI). The aim of this study was to clarify the relationship between factor V Leiden and prothrombin G20210A with acute MI in patients younger than 50 years of age. Materials and methods: In this case-control study we recruited 101 MI patients and 101 healthy individuals in Fatemeh Zahra Hospital, affiliated to Mazandaran University of Medical Sciences. Demographic information and risk factors including age, past history of MI, diabetes mellitus, hypertension, dyslipidemia, and smoking were recorded. Genes of factor V Leiden and prothrombin G20210A were analyzed using PCRRFLP method. Results: Among all subjects, 191 (94.55%) had no factor V Leiden gene, 10 (4.95 %) were heterozygous, and one (0.49%) was homozygous. In case group, and control group there were seven (6.9%) and three (2.97%) heterozygous for factor V Leiden, respectively (P= 0.27). There was no mutation of prothrombin G20210A gene in 197 (97.5%) individuals and only five (2.5%) were heterozygous. There was no statistically significant difference between the two groups (P= 0.17). Conclusion: The prevalence of factor V Leiden and prothrombin G20210A gene mutation, in patients was not significantly different from that of the normal individuals. However, further studies are recommended in this age group to investigate these factors.
