درك پرستاران از رفتارهاي مراقبتي در بخش‌هاي ويژه بيمارستان‌هاي دانشگاه

Nursesʹʹ perception of caring behaviors in intensive care units in hospitals of Lorestan University of Medical Sciences, Iran

زمينه و هدف: مراقبت هسته اصلي پرستاري است، اما درك متفاوتي از آن وجود دارد. بررسي و اندازه‌گيري مداوم رفتارهاي مراقبتي سبب يافتن اشكالات آن مي‌شود. طرح‌ريزي مداخلات متفكرانه و رفع اشكالات باعث بهبود مراقبت خواهد شد. اين مطالعه با هدف تعيين درك پرستاران بخش‌هاي ويژه از رفتارهاي مراقبتي انجام گرفت. مواد و روش‌ها: در مطالعه توصيفي- تحليلي حاضر كه در سال 1391 انجام شد، 140 پرستار از بخش‌هاي مراقبت ويژه بيمارستان‌هاي دانشگاه علوم پزشكي لرستان به صورت سرشماري انتخاب شدند. ابزار جمع‌آوري اطلاعات، پرسش‌نامه دو قسمتي CBI‏ (‏Caring inventory behaviour‏) ‏شامل اطلاعات دموگرافيك و 28 گويه مربوط به مراقبت بود. اعتبار ترجمه به صورت صوري و اعتبار محتوايي پرسش‌نامه با نظر متخصصين فن تامين و پس از حذف 4 گويه، فرم نهايي پرسش‌نامه با 24 گويه تهيه شد. به منظور بررسي پايايي، ضريب Cronbach?s alpha برابر با 71/0 به دست آمد. در نهايت داده‌ها با استفاده از نرم‌افزار SPSS نسخه 18 و آزمون‌هاي آمار توصيفي و تحليلي (كروسكال واليس و من ويتني) تحليل شد. يافته‌ها: پرستاران بيشتر به جنبه جسماني- تكنيكي (76/12 ± 71/95) در مقايسه با بعد رواني- اجتماعي (91/27 ± 41/75) مراقبت توجه داشتند و حداكثر نمره آنان به رفتار مراقبتي «به موقع انجام دادن اقدامات درماني و دارو دادن» اختصاص داشت. نتيجه‌گيري: از آن‌جا كه پرستاران شركت كننده به جنبه‌هاي معمول تكنيكي مراقبت بيشتر از جنبه‌هاي عاطفي- اجتماعي اهميت دادند، با فراهم نمودن درك پرستاران از مراقبت مي‌توان مراقبتي متناسب با نيازهاي بيماران ارايه نمود. از اين طريق نه تنها موجبات رضايت هرچه بيشتر بيماران از مراقبت‌هاي پرستاري فراهم مي‌شود، بلكه به طور غير مستقيم منجر به ايجاد نگرش مثبت بيماران و جامعه نسبت به حرفه پرستاري و خدمات آن مي‌گردد. همچنين مسوولان آموزش حرفه پرستاري با استفاده از اين نتايج مي‌توانند دانشجويان را در كار باليني جهت برآورده نمودن نيازها و انتظارات بيماران سوق دهند. كليد واژه‌ها: رفتار مراقبتي، درك پرستاران، بخش‌هاي مراقبت ويژه

Background and Objective: Caring is the core of nursing; however, different individules have different perceptions of it. Continuous assessment and measurement of caring behaviors results in the identification of their problems. The careful planning of interventions and problem solving will improve care. The aim of this study was to identify nursesʹ perception of caring behaviors in the intensive care units. Materials and Method: In this descriptive-analytic study, 140 nurses were selected from intensive care units of hospitals affiliated to Lorestan University of Medical Sciences, Iran, using the census method in 2012. The data collection tool was the Caring Behaviors Inventory for Elders (CBI-E). This questionnaire consisted of two parts including demographic information and 28 items related to care. Face and content validity of the Persian version of the questionnaire were provided by professionals, and after deletion of 4 items a 24-item questionnaire was provided. Cronbachʹs alpha coefficient was calculated to assess reliability (? = 0.71). Data were analyzed using SPSS software version 18 and descriptive-analytic statistics (Kruskal-Wallis test and Mann-Whitney test). Results: Based on the findings, nurses paid more attention to the physical–technical aspects (95.71 ± 12.76) of care in comparison to its psychosocial aspects (75.41 ± 27.91). Nurses had the highest score in care behavior of "timely performance of medical procedures and medication administration". Conclusion: Since nurses paid more attention to the technical aspects of care than its psychosocial aspects, by providing nurses with a correct perception of care, patients can be provided with needs-based care. This will increase patient satisfaction with nursing care, and indirectly result in the positive attitude of patients and society toward the nursing profession and its services. Moreover, nursing education officials can use these results to assist nurses in meeting patientsʹ needs and expectations in clinical practice.
