بررسي ضخامت آندومتر در بيماران آمنوره مبتلا به سرطان پستان تحت درمان با تاموكسيفن

چكيده زمينه و هدف: سرطان پستان يكي از سرطان هاي شايع زنان است كه از درمان‎هاي رايج آن مصرف تاموكسيفن است. يكي از عوارض ذكر شده تاموكسيفن افزايش ضخامت آندومتر و خطر ابتلا به سرطان اندومتر است. هدف اين مطالعه بررسي اثر تاموكسيفن بر ضخامت اندومتر در بيماران مبتلا به سرطان پستان مصرف كننده تاموكسيفن كه دچار آمنوره شده‎اند مي‎باشد. مواد وروش ها: اين مطالعه مقطعي- تحليلي به مدت 7 سال در بخش‎هاي پرتو درماني دانشگاه علوم پزشكي مشهد، انجام شد. 162بيمار مبتلا به سرطان پستان تحت درمان با تاموكسيفن كه قاعدگي آنها قطع شده بودند، انتخاب شدند و ارتباط بين مصرف تاموكسيفن و ضخامت آندومتر بررسي شد. هم‎چنين ارتباط بين مدت مصرف تاموكسيفن و شيوع ضخامت غير طبيعي اندومتر بررسي شد. ارتباط متقابل بين خونريزي غير طبيعي واژينال و يافته‎هاي سونوگرافي هم مورد ارزيابي قرار گرفت. سپس بررسي آماري با استفاده از آزمون‎هاي كاي اسكوير و آزمون تي انجام شد. يافته ها : بين مصرف تاموكسيفن و ضخامت آندومتر ارتباط وجود داشت و بيشترين ارتباط در سال اول درمان بود. ارتباط بين خونريزي غير طبيعي واژينال و سونوگرافي غير طبيعي آندومتر نيز معني‎دار بود (001/0=p). شيوع سرطان آندومتر در مصرف كنندگان تاموكسيفن بيشتر از جمعيت عمومي بود (66/0 درصد در مقابل 1/0 درصد). نتيجه گيري: با توجه به نتايج به دست آمده، در مصرف كنندگان تاموكسيفن در صورت علايم باليني دال بر خونريزي غير طبيعي واژينال و يا وجود سونوگرافي غير طبيعي بايد كورتاژ آندومتر و بررسي آسيب شناسي اندومتر انجام شود. واژگان كليدي: آمنوره، بيوپسي آندومتر، تاموكسيفن، سرطان پستان، سرطان آندومتر، ضخامت آندومتر

Abstract Background: Breast cancer is one of the most common cancers in women worldwide and one of its most desirable treatments is tamoxifen. The reported side effect associated with tamoxifen is endometrial thickness and increased risk of endometrial cancer. The aim of this study was to assess the influence of tamoxifen on the endometrial thickness in amenorrhea breast cancer patients treated with tamoxifen. Materials and Methods: This descriptive-analytic, cross-sectional study was conducted in Radiotherapy Departments of Mashhad University of Medical Sciences over a period of seven years.162 patients treated with tamoxifen whose bleeding cycle had been disrupted were selected and the relationship between tamoxifen usage and endometrial thickness was investigated in them. In addition, the relationship between abnormal vaginal bleeding and the ultra sonographic findings was assessed. Data analysis was done by using t-test and Chi-square test. Results: There was a significant relationship between tamoxifen usage and abnormal endometrial thickness while the greatest relationship was observed in the first year of treatment. The relationship between abnormal vaginal bleeding and abnormal endometrial sonographic findings was significant (P=0.001). The incidence of endometrial cancer in tamoxifen users was more than general population (0.61% vs. 0.1%). Conclusion: Considering the findings of this study, in the presence of abnormal sonographic and clinical observations indicating abnormal vaginal bleeding, pathological evaluation of the endometrial biopsy should be carried out for patients who use tamoxifen.
