رويكردهاي ارتقاي همكاري بين بخشي در سلامت از طريق شوراي عالي سلامت و امنيت غذايي كشور

Improving Approaches of Intersectoral Collaboration for Health by Health and Food Security High Council in I.R.Iran

زمينه و هدف: همكاری بين بخشی و مشاركت مردم دو راهبرد اصلی در برقراری عدالت در سلامت است. مطابق قانون برنامه چهارم توسعه، شورای عالی سلامت و امنيت غذايی كشور(شورای عالی) به عنوان سياستی برای توسعه همكاری بين بخشی شكل گرفت. بررسی ديدگاه‌های اعضای كميسيون دايمی اين شورای عالی می تواند مسير روشنی را پيش روی مسؤولان ارشد وزارت بهداشت، برای ارتقای سلامت مردم گذارد. روش كار: مطالعه توصيفی و كيفی بود. اطلاعات از طريق مصاحبه ساختارمند با اعضای كميسيون دايمی شورای عالی و تحليل محتوای اسناد مرتبط به خاستگاه و عملكرد آن از بدو تاسيس گردآوری شد. مصاحبه ها با تعيين قلمروهای اصلی تحليل و جمع بندی شده اند. نتايج: اغلب اعضای كميسيون دايمی اثر بخشی شورای عالی را كم و خيلی كم می دانند. تعيين اولويت های سلامت و سپس تكاليف يا انتظارات از هر دستگاه به عنوان اولين گام پيشنهاد شد. الزام دستگاه ها در معرفی يك نماينده دائم و ثابت، ايجاد كارگروه‌های كارشناسی برای پخته شدن موضوعات، تشكيل جلسات منظم در دبيرخانه و اطلاع رسانی مستمر به اعضا، تقويت ساختار متناظر در استان ها و تفويض اختيار به آنان و وصل شدن شورا به كميته علمی ثابت از نخبگان سلامت از جمله راهكارهای پيشنهادی بود. نتيجه گيری: عملكرد شورای عالی با اثربخشی مورد انتظار قانون گذار فاصله دارد. استقرار دبيرخانه اين شورا با نيروی كارشناسی و نظام كاری ويژه «همكاری بين بخشي» تا حد زيادی ضعف ها را پوشش می دهد. تعريف نقش ها و انتظارات دوره‌ای از دستگاه ها، توانمندسازی رابطان سلامت وزارت خانه ها و سازمان ها و تشويق دستگاه هايی كه مشاركت موثر داشته اند، همكاری را مستحكم تر و مداوم تر می‌كند.

Background and Aim: Intersectoral Collaboration and Community Participation are two main strategies for sustaining equity in health. Based on the 4th 5years country development plan, Health and Food Security High Council(HC) as a strategy for developing intersectoral collaboration. Reviewing membersʹ opinions of HC may show the clear way of improving intersectoral collaboration for health promotion of the population to senior officials of Ministry of Health and Medical Education. Materials and Methods: This was a descriptive and qualitative study. Information were gathered by structured interview to HC members and content analysis of the documents and gray materials related to initiation and performance of the HC since itʹs opening. Content analysis and adding up the interviews were done by hand after defining the main domains. Results: Most of HC members believed that the HC effectiveness is very low or low. For the 1st step, defining health priorities and then responsibilities and expectations of each organization were proposed. Organizations necessity to introduce a sustained representative, creating taskforces for deep working on the issues, regular meeting in secretariat, concurrent information sharing to the members, strengthening similar structure in provinces and giving more authority to them and connecting the council to the scientific committees of health elites are main improvement approaches. Conclusion: There is gap between the performance of the HC and the law makersʹ expectations. Implementing secretariat of HC including expertise and special working system of intersectoral collaboration will mostly cover the weakness. Defining periodical roles and expectations of organizations and capacity building of health collaborators of the ministries and organizations, and incentivizing organization which have effective contribution, will strengthen and continue the collaboration more.
