بررسي ارتباط بين ضخامت آندومتر در سونوگرافي واژينال با ميزان پاسخ به درمان متوتركسات عضلاني تك دوز در بيماران حاملگي خارج رحمي

Evaluation of the relationship between endometrial thickness measured by transvaginal ultrasound and response rate to the treatment with single-dose intramuscular methotrexate in the women with ectopi

چكيده سابقه و هدف: حاملگي خارج رحمي يك اورژانس ژينكولوژي به معناي جايگزيني محصولات حاملگي در خارج از كانال آندومتر است و اثرات زيانبار اقتصادي، اجتماعي و فردي فراواني دارد. هدف اين مطالعه تعيين ارتباط بين ضخامت آندومتر در سونوگرافي واژينال با ميزان پاسخ به درمان متوتركسات عضلاني تك دوز در بيماران حاملگي خارج رحمي است. روش بررسي: اين مطالعه از نوع مقطعي است و در 63 بيمار كه BHCG بالاتر يا مساوي mIU/mL 1200 بوده و در سونوگرافي واژينال آنها ساك حاملگي داخل رحمي مشاهده نشد، همچنين با سطح سرمي ثابت BHCG و يا افزايش كمتر از 50% در طي 48 ساعت به بيمارستان امام رضا مراجعه نمودند، ضخامت آندومتر اندازه گيري شد. سپس، متوتركسات عضلاني به ميزان mg/m2 50 تجويز شده و تيتر BHCG در روزهاي 4 و 7 بعد از شروع درمان اندازه گيري شد. داده ها با استفاده از آزمون‌هاي آماري و نرم افزار SPSS.21 تجزيه و تحليل گرديد. يافته ها: درصد موفقيت درمان با متوتركسات عضلاني تك دوز برابر 5/82 درصد بود. بين سن، سن حاملگي و اندازه توده با ضخامت آندومتر ارتباط معناداري وجود نداشت (05/0P > ). اما، بين ميزان HCGB و ضخامت آندومتر ارتباط معناداري وجود داشت (05/0P < ). بين ضخامت آندومتر با ميزان پاسخ به درمان متوتركسات عضلاني تك دوز ارتباط معناداري وجود داشت (05/0P < ). نتيجه گيري: بر اساس نتايج اين مطالعه، ضخامت آندومتر با ميزان پاسخ به درمان متوتركسات عضلاني تك دوز ارتباط معناداري دارد، لذا بهره گيري از پارامتر ضخامت آندومتر در سونوگرافي واژينال بيماران حاملگي خارج رحمي مي تواند مثمرثمر باشد. واژه هاي كليدي: حاملگي خارج رحمي، BHCG، آندومتر، سونوگرافي واژينال.

ABSTRACT Background and Aim: Ectopic pregnancy is an emergency gynecologic problem which means replacement of blastocyst outside the uterine cavity and it has many detrimental effects in regard to social, economic and personal aspects. The aim of this study was to determine the relationship between endometrial thickness measured by transvaginal ultrasound and response rate to the treatment of ectopic pregnancy with single-dose intramuscular methotrexate. Material and Method: This cross-sectional study was conducted in Imam Reza Hospital, in Kermanshah, and included 63 patients with BHCG greater than or equal to 1200 mIU/mL and without ultrasonography evidence of intrauterine gestational sac. Also patients with constant BHCG level or increased levels of less than 50% within 48 hours were regarded as cases of ectopic pregnancy. Then endometrial thickness was measured. Methotrexate 50 mg/m2 was administered intramuscularly. BHCG titers were measured 4 and 7 days after treatment. Using SPSS 21 software, data were analyzed by statistical tests. Result: We achieved a treatment success of 82.5% with single dose intramuscular methotrexate. Age and size of mass had no relationship with endometrial thickness (p > 0.05). However, there was a correlation between endometrial thickness and BHCG titer (p < 0.05). We also found a significant relationship between endometrial thickness and the response rate to the treatment of ectopic pregnancy with single-dose intramuscular methotrexate. Conclusion: According to the results of this study, endometrial thickness has a significant relationship with the response to therapy with single dose intramuscular methotrexate. Therefore, endometrial thickness, measured by transvaginal ultrasound, can be used as a parameter in ectopic pregnancy. Keywords: Ectopic Pregnancy, BHCG, Endometrial, Transvaginal ultrasound.
