اعتبار و تكرارپذيري Dual Digital Inclinometer در اندازه گيري زاويه كايفوز پشتي در زنان بالاي 45 سال

Validity and Reproducibility of Dual Digital Inclinometer in measuring thoracic kyphosis in women over 45 years

هدف: اندازه گيری دقيق كايفوز پشتی ستون فقرات هم در روند ارزيابی بيماران و هم در سنجش اثر درمانهای مختلف از ارزش بالايی برخوردار است.Dual Digital Inclinometer(DDI) به عنوان ابزاری كه می تواند به صورت غير تهاجمی مقدار زاويه كايفوز را اندازه گيری كند معرفی شده است. امابنا بر اطلاعات موجود تاكنون تنها يك مطالعه به بررسی اعتبار و تكرارپذيری اين ابزار دركشورمان پرداخته است. لذا هدف از اين مطالعه بررسی اعتبار وتكرارپذيری درون روز وبين روز ابزار DDI دراندازه گيری زاويه كايفوز پشتی ستون فقرات در جامعه زنان بالای 45 سال می باشد. روش بررسی : در اين مطالعه متدلوژيك ، كايفوز پشتی ستون فقرات توسط ابزار DDI در سه نوبت بر روی 20زن در دامنه سنی 70-45 اندازه گيری شد.دو اندازه گيری در يك روز به فاصله يك ساعت برای بررسی تكرارپذيری درون –روز و نوبت آخر با فاصله يك روز برای ارزيابی تكرارپذيری بين- روزها صورت گرفت.همچنين مقدارزاويه كايفوز با محاسبه زاويه Cobb بر روی تصاويرراديولوژيكی توسط آزمونگر اندازه گيری شد و اعتبارزاويه به دست آمده با ابزار DDI در مقايسه با آن مورد بررسی قرار گرفت.از آزمون ضريب همبستگی ICC با فاصله اطمينان 95% جهت تجزيه و تحليل داده ها استفاده شد. يافته ها : دربررسی اعتباراندازه گيری كايفوز به دست آمده با ابزار DDI در مقايسه با زاويه Cobb مقدار 86/0ICC= به دست آمد كه سطح اعتباربالايی را نشان می دهد. همچنين تكرارپذيری درون روز و بين روز زاويه كايفوز پشتی اندازه گيری شده با ابزار DDI به ترتيب با ضريب همبستگی 95 /0ICC= و92/0ICC=به دست آمد. نتيجه گيری : بنابر يافته های اين پژوهش، ابزار DDI می تواندبه عنوان وسيله ای معتبر و تكرارپذير برای اندازه گيری غيرتهاجمی كايفوز پشتی ستون فقرات به كارگرفته شود.

Objective: Accurate measurement of thoracic kyphosis has high value in the evaluation of patients with spinal kyphosis and in assessing the effectiveness of different treatment methods. Recently, Dual Digital Inclinometer (DDI) has introduced as a non-invasive tool that can measure the kyphosis angle. According to available information there is only one published study regarding the validity and reliability of this instrument in our country. Therefore, this study aimed to evaluate the Validity and reproducibility of DDI in measuring thoracic kyphosis in women over 45 years. Materials and Methods: In this methodological study, thorasic kyphosis was measured by DDI device three times on 20 women aged 45-70 years. The first two measurements were taken with an hour apart in same day to assess the within- day and the third measurement was taken one day later to assess the between-days reproducibility. Finally, the validity of obtained values of thorasic khyphosis by DDI compared with Cobb angle which measured by the same examiner on radiological films. ICC correlation coefficient test with 95% confidence intervals was used for data analysis. Results: The validity of values obtained by DDI compared to the Cobb angle, as a gold standard, was high (ICC=0.86). Within-day and between-days thorasic kyphosis measurements using DDI were shown to be reproducible with high ICC values. (0.95 for within-day and 0.93 for between days respectively). Conclusion: According to the present study, the DDI is a valid and reproducible tool for non-invasive measurement of thoracic kyphosis in women over 45 years.
