مقايسه الگوي رشد كودكان مبتلا به كم كاري مادرزادي تيروييد شهر يزد با كودكان سالم در سال 1393: كوهورت تاريخي

Comparison of the growth patterns among children with congenital hypothyroidism and healthy children in Yazd city in 2014: a historical cohort study

سابقه و هدف: كم كاري مادرزادي تيروييد يكي از شايع ترين بيماري هاي غدد درون ريز مي باشد كه مي تواند بر روي رشد كودكان مبتلا تاثير بگذارد. پژوهش حاضر با هدف بررسي مقايسه الگوي رشد كودكان مبتلا به كم كاري مادرزادي تيروييد و كودكان سالم شهر يزد انجام شد. مواد و روش ها: مطالعه تحليلي حاضر به صورت كوهورت گذشته نگر (Retrospective Cohort) بر روي كليه نوزادان متولد سال-هاي 1385 تا 1387 كه توسط طرح غربال گري به عنوان بيمار تشخيص داده شده بودند، انجام شد. ميانگين قد، وزن و دور سر به همراه نسبت اختلال رشد هريك از اين متغيرها طي 5 سال ابتداي زندگي در كودكان مبتلا و كودكان سالم شهر يزد به تفكيك سن و جنس مورد مقايسه قرار گرفت. نتايج: در هيچ مقطع سني اختلاف معني داري بين ميانگين قد، وزن و دور سر دختران بيمار و سالم مشاهده نشد، اما ميانگين قد پسران بيمار در سن 3 تا 24 ماهگي، ميانگين وزن شان در سن 6 تا 18 ماهگي و مبانگين دور سر آنها در سن 12 تا 18 ماهگي اختلاف معني-داري با پسران سالم شهر يزد داشت. در پايان 5 سال پيگيري هيچ گونه اختلاف معني داري از لحاظ نسبت اختلال رشد قد، وزن و دورسر بين كودكان سالم و بيمار دو جنس مشاهده نگرديد. نتيجه گيري: پس از درمان بيماران مبتلا و با گذشت دوره مراقبت آنها مي توان اميدوار بود الگوي رشد اين كودكان در محدوده طبيعي و مطلوب قرار گيرد.

Background: The congenital hypothyroidism is one of the most common endocrine diseases, which can affect the growth of the children. This study aimed to compare the growth patterns among the children with congenital hypothyroidism (CH) and healthy children in Yazd, Iran. Materials and Methods: This retrospective cohort study was performed on all neonatal children with CH, who had been born since 2006 to 2008. The average height, weight and head circumference along with the growth disorder ratios for these variables were measured and compared in the children with CH and healthy children during the first 5 years of life after considering their age and gender. Results: Results showed no significant difference in the average height, weight and head circumference between the girls with CH and the healthy ones. Moreover, there was a significant difference in the average height of boys at the age of 3 to 24 months, the average weight at the age of 6 to 18 months and the average head circumference at the age of 12 to 18 months compared with the values of healthy children in this city. At the end of a 5-year follow-up, no significant difference was observed in the growth disorder ratio for weight, height and head circumference between both the healthy children and patients in both genders. Conclusion: After treatment and care for these patients, the growth patterns of these children would be within the normal and desirable range.
