مشاركت كاربران در توسعه سيستم‌هاي اطلاعات بيمارستاني و عوامل موثر بر آن

USER PARTICIPATION IN THE DEVELOPMENT OF HOSPITAL INFORMATION SYSTEMS AND FACTORS AFFECTING IT

پيش‌زمينه و هدف: با توجه به حجم انبوه اطلاعات توليدشده در بيمارستان‌ها، براي اداره كارآمد آن‌ها بهره‌گيري از سيستم اطلاعات بيمارستاني (HIS) امري ضروري است. در اين ميان يكي از عوامل اصلي در موفقيت HIS، مشاركت كاربران هست كه باعث شده تا نيازهاي اطلاعاتي و فرآيندي به‌درستي پيش‌بيني‌شده و همچنين تعهد آن‌ها نسبت به توسعه HIS افزايش يابد. هدف اين تحقيق، بررسي ميزان مشاركت كاربران در مراحل مختلف توسعه HIS و همچنين شناسايي عوامل موثر بر آن هست. مواد و روش كار: اين مطالعه از نوع توصيفي-تحليلي بود كه به روش مقطعي در سال 1392 انجام گرفت. جامعه پژوهش شامل كاربران HIS در مراكز آموزشي درماني دانشگاه علوم پزشكي اروميه بود كه به روش نمونه‌گيري خوشه‌اي چندمرحله‌اي، 140 نفر از آن‌ها انتخاب شد. داده‌ها از طريق يك پرسشنامه كه روايي و پايايي آن مورد تاييد قرار گرفت، جمع‌آوري گرديد. داده‌هاي جمع‌آوري‌شده توسط نرم‌افزار آماري SPSS و با استفاده از آزمون‌هاي خي دو و t-test تحليل شد. يافته‌ها: بيشترين ميزان مشاركت كاربران در مراحل 4 گانه توسعه HIS مربوط به مرحله پياده‌سازي (2.88) و كمترين ميزان مشاركت در مرحله تحليل (1.23) هست. نتايج آزمون آماري نشان داد كه در هيچ‌كدام از مراحل توسعه (تحليل، طراحي، پياده‌سازي و ارزيابي) ميزان مشاركت كاربران در حد مطلوبي نبودند. (P < 0.05) تشكيل گروه كاري از كاربران نهايي و حمايت مديران رده بالا از توسعه HIS از مهم‌ترين عوامل موثر بر افزايش ميزان مشاركت هست. بحث و نتيجه‌گيري: با توجه به نتايج اين تحقيق، ضروري به نظر مي‌رسد مديران مراقبت سلامت قبل از توسعه و خريداري HIS، برنامه‌ريزي دقيق جهت مشاركت كاربران انجام دهند تا نيازهاي واقعي شناسايي و همچنين تعهد و انگيزه آن‌ها در توسعه، نگهداري و ارتقاي سيستم افزايش يابد تا منجر به تضمين موفقيت سيستم گردد.

Background &Aims: Due to the large volume of the data generated in hospitals, it is essential to use hospital information system (HIS) in efficiently handling them. One of the major factors in the HIS is user participation that leads to correctly predicted factors and this fact also increase their commitment to the development of HIS. This study aimed to survey participation rate of users in different stages of HIS development and identified factors affecting it. Materials & Method: This descriptive-cross sectional study was performed on 140 users (including physicians, nurses, medical laboratorists, radiologists, HIM and IT staffs) from Urmia University Hospitals in 2013. The data were collected using a self-structured questionnaire whose reliability had been confirmed by specialists as well as its validity by internal consistency method (r =0.86). The collected data were analyzed by SPSS software using descriptive statistics and analytical statistics (t-test and chi-square). Results: Correlation analysis showed that a meaningful correlation existed between job and organizational change barriers (p =0.003), and also between the history and attitudinal-behavioral barriers (p =0.006). The highest participation rate of users in the four-stage development of the HIS was related to implementation phase (2.88) and the lowest participation rate was related to analysis (1.23). The test results showed that none of the stages of development (analysis, design, implementation, and evaluation) had satisfactory user participation (P < 0.05). The most important factors in increasing user participation included established teamwork from end-users and the support of top managers from HIS development. Conclusion: According to the survey results, it seems necessary that health care administrators have a detailed plan for user participation that identify the real needs prior to the development and purchase of HIS, and predict ways to increase users’ commitment and motivations to develop maintain and upgrade the system.
