بررسي همبستگي بين آزمون‌هاي تعادل و تحرك عملكردي و پارامترهاي نوسان مركز فشار در حالت ايستاده آرام با و بدون تكليف شناختي در كودكان با فلج مغزي همي‌پارزي

Correlation between clinical and laboratory tests of mobility and balance in children with hemiparetic cerebral palsy

چكيده مقدمه: به دليل تاثير شخص، تكليف و محيط بر روي كنترل وضعيتي كودكان با فلج مغزي همي‌پارزي، روش‌هاي مختلف باليني و آزمايشگاهي براي سنجش كنترل وضعيتي مطرح است. هدف از اين مطالعه بررسي همبستگي ميان آزمون‌هاي باليني و آزمايشگاهي تعادل در كودكان با فلج مغزي همي‌پارزي بود. مواد و روش‌ها: در اين مطالعه همبستگي، 28 كودك فلج مغزي همي‌پارزي با متوسط سني(34/2 ± 09/10) سال به‌روش نمونه‌گيري غير احتمالي ساده انتخاب شدند. آزمون‌هاي Timed Up & Go (TUG)، Functional Reach (FR)، Bruininks Oseretsky Test of Motor Proficiency (BOTMP) و پارامترهاي سرعت متوسط، طول مسير، صفحه فاز كل و سطح كل نوسانات مركز فشار در سه سطح سختي وضعيتي (سطح سفت با چشم باز و بسته و سطح فوم با چشم بسته در حالت ايستاده آرام) و دو سطح سختي‌شناختي (با و بدون تكليف شناختي) ثبت‌شده با صفحه نيرو به‌ترتيب به‌عنوان آزمون‌هاي باليني و آزمايشگاهي استفاده شد. يافته‌ها: از ميان 3 تكليف وضعيتي فقط در ايستادن روي فوم با چشم بسته همبستگي متوسط تا بسيار بالايي بين تمامي پارامترهاي نوسان مركز فشار و آزمون‌هاي TUG، FR و BOTMP ديده شد. تكليف‌شناختي در حالت ايستادن روي سطح فوم با چشم بسته باعث افزايش همبستگي بين آزمون‌هاي باليني و آزمايشگاهي گرديد. نتيجه‌‌گيري: همبستگي مناسبي بين آزمون‌هاي تعادل و تحرك عملكردي و پارامترهاي نوسان مركز فشار در حالت ايستادن روي سطح فوم با چشم بسته به‌دست آمد. كليد واژه‌ها: كنترل وضعيتي، آزمون‌هاي تعادل و تحرك عملكردي، پارامترهاي صفحه نيرو، كودكان فلج مغزي، همي‌پارزي

Abstract Introduction: Children with hemiparetic cerebral palsy (HCP) experience a permanent disorders in balance and protective reactions and lead to postural control deficiencies in their everyday activities. The purpose of this study was to investigate the correlation between clinical and laboratory tests of mobility and balance in children with HCP. Materials and methods: In this study twenty eight children with HCP (Mean age±SD; 10.09±2.34 years) were selected by simple non-probability sampling method. Timed Up and Go (TUG), Functional Reach (FR) tests and Bruininks Oseretsky Test of Motor Proficiency (BOTMP), mean velocity, path length, total phase plane, area parameters of center of pressure fluctuations were measured. All tests were collected in 3 levels of postural difficulty (quiet standing on rigid surface with open and closed eyes and foam with closed eyes) and two levels of cognitive difficulty (with and without cognitive task) as clinical and laboratory assessments respectively. Force plate was used to recorded all data. Results: Among three postural tasks, a significant moderate to very high correlation between all center of pressure sway measures and TUG, FR and BOTMP tests in standing on foam with closed eyes was observed. It seems the cognitive task in standing on foam surface condition caused to increase the correlation between clinical and laboratory measures. Conclusion: Based on results, the proper correlation was obtained between functional balance and mobility tests and center of pressure sway parameters in standing on foam with closed eyes.
