Survey and Calculating the Bed-Day cost and Day-Patient Cost of Orthopedi Department of Khomeiny Hospital ofTehran University of Medical Science by Using Activity Based Costing (ABC) Method -2008

زمينه و هدف: يكی از مهم ترين مزيت‌های محاسبه قيمت تمام شده و هزينه تخت روز آن است كه دولت می تواند در مراكز گوناگون و مناطق مختلف كشور اين قيمت تمام شده را مورد مقايسه و تجزيه و تحليل قرار داده و بر مبنای آن بودجه و برنامه دقيقی تنظيم و واحد های دولتی را به اجرای آن ملزم سازد. مواد و روش ها: اين پژوهش به شيوه توصيفی- تحليلی به صورت مقطعی و گذشته نگر اجرا می شود. انتخاب نمونه (بخش ارتوپدی مردان) بنا به نياز مسئولين بيمارستان- در راستای پاسخ به اين سوال كه آيا بخش ارتوپدی مردان درآمدزاست؟- با استفاده از نظر كارشناسان محترم انجام گرديده است. داده های مربوط به اين مطالعه از طريق مطالعه اسناد و مدارك، مصاحبه و مشاهده در چك ليست های مربوطه جمع آوری گرديد. ابتدا كليه مراكز فعاليت در بيمارستان مشخص و سپس بر اساس تكنيك ABC هزينه هر فعاليت محاسبه شد .يافته ها: هزينه تخت روز اشغالی بدست آمده برای بخش ارتوپدی مردان بيمارستان امام خمينی (ره) در سال 1387(326,818 ريال) است. با عنايت به اين موضوع كه درآمد اين بخش از طريق فروش خدمات بر اساس تعرفه مصوب وزارت بهداشت درمان و آموزش پزشكی تأمين می شود و اين رقم برای هتلينگ اتاق دو تخته بيمارستان‌های دولتی درجه يك شهر تهران در سال 1387 (217500 ريال) است می توان گفت هزينه تخت روز اشغالی در مقايسه با تعرفه مصوب آن سال ، 109318 ريال بيشتر است كه اين تفاوت بيانگر آن است كه رقم تعرفه با تفاوت زيادی كمتر از هزينه تخت روز می باشدنتيجه گيری: اختلاف زياد بين قيمت تمام شده، تخت روز(هتلينگ) و تعرفه مصوب هئيت دولت منجر به زيان دهی مراكز ارائه دهنده خدمت می گردد و در بلند مدت ارائه خدمت با كيفيت را با مشكل مواجه خواهد ساخت. لذا تعيين تعرفه های خدمات تشخيصی درمانی براساس قيمت تمام شده واقعی و به صورت علمی ضروری به نظر می رسد.

Background: One of the most important advantages of calculating the cost and the bed-day expense is that the government can analyze and compare this cost among different centers and regions of the country and adjust an accurate plan and budget and require governmental units to perform it.Materials and Methods: this research has been done using a descriptive-analytic with a temporary and retrospective approach. sample selection has been done regarding the question that "whether the orthopedic section is beneficial or not" -based on hospital chief`s demands and with expertsconsultation. Related data has been gathered through studying documents, performing interviews and observing related checklists .initially all of the active centers were verified and then each activity`s cost was calculated using the ABC technique.Results: in the year 1387, cost of each occupied bed for the orthopedic section of Imam Khomeini hospital has been 326818 rials per day. Considering the issue that this sector`s income is provided by selling services based on approved tariffs of ministry of health and medical education which has been 217500 rilas for a double-bed room of first grade hospitals in Tehran in the same year, we could conclude that the occupied Bed-Day cost is 109318 rials more compared to the approved tariff of the targeted year, which shows that the tariffʹs value is highly less than the bed-day cost.Conclusion: The enormous variance between Bed-Day cost and the tariff approved by the iranian cabinet will result in forfeit of service providing centers and this could make them serious problems in providing quality services in long term. Therefore, it is essential to set the diagnostic and treatment serviceʹs tariffs based on the real cost of these kind of services.
