چالش‌هاي اخلاقي در داروخانه از ديدگاه اساتيد دانشكده‌ي داروسازي و داروسازان شاغل در داروخانه‌هاي شهر شيراز: مطالعه‌ي كيفي

Ethical challenges in the community pharmacy setting from the perspective of the faculty members of Shiraz School of Pharmacy and pharmacy practitioners: a qualitative study

داروسازان، به‌عنوان اعضای جامعه به‌طور كل و هم‌چنین، به‌عنوان اعضای كادر سلامت، در بسیاری از فعالیت‌هایی شركت دارند كه می‌تواند ایشان را در معرض چالش‌های اخلاقی قرار دهد. آن‌ها در حین انجام كار خود ممكن است با مشكلات و مسائلی روبه‌رو شوند كه آیا آنچه انجام داده‌اند، درست، مناسب و قانونی است یا نه؟ این مطالعه به بررسی چالش‌های اخلاقی از دیدگاه داروسازان داروخانه و اساتید دانشكده‌ی داروسازی شیراز می‌پردازد و جنبه‌ی تفسیری و فرایندهای استدلالی آن‌ها از پدیده‌های اجتماعی مانند مسائل اخلاقی و تصمیم‌گیری اخلاقی برای مقابله با چنین مشكلاتی در داروخانه‌ها و در تعاملات بین داروساز با افرادی كه با آن‌ها رابطه دارد را مورد بررسی قرار می‌دهد. پژوهش حاضر یك مطالعه با رویكرد كیفی، از نوع تحلیل محتواست. شركت‌كنندگان 20 نفر از داروسازان از جامعه‌ی آماری داروسازان شاغل به كار در داروخانه‌های شهر شیراز و اساتید دانشكده‌ی داروسازی بودند. داده‌ها از طریق مصاحبه‌های نیمه‌ساختارمند و با نمونه گیری مبتنی بر هدف جمع‌آوری و تا اشباع داده‌ها ادامه یافت.. تجزیه و تحلیل داده‌ها با رویكرد تحلیل محتوای كیفی انجام شد. تجزیه و تحلیل مصاحبه‌ با شركت‌كنندگان در مطالعه، به استخراج شش مقوله شامل درك داروسازان از مفاهیم كلیدی اخلاق، وظایف و تجربیات داروسازان، روابط با سایر كادر سلامت، داروخانه به‌عنوان یك مكان اقتصادی، تفاوت رعایت اخلاق در داروسازی با پزشكی و پرستاری از دیدگاه داروسازان و چگونگی پاسخ عملی داروسازان در مواجهه با مشكلات اخلاقی، منتج شد. هر كدام از این مقوله‌‌ها، نیز به زیر مقوله‌های مربوط به موارد خاص چالش‌های اخلاقی در محیط كار، خوشه‌بندی شد. آگاهی از مسائل اخلاقی و دستورالعمل‌های حرفه‌ای، یك بخش اساسی در آموزش ارائه‌دهندگان خدمات سلامت است. اهداف گسترده از آموزش اخلاق رسمی در افراد حرفه‌ای، بالا بردن آگاهی آن‌ها از مسائل اخلاقی و دستورالعمل‌های حرفه‌ای و ارائه‌ی یك ساختار برای ارزیابی مشكلات اخلاقی و تصمیم‌گیری مناسب و قابل توجیه است. یافته‌های این مطالعه نشان‌دهنده‌ی نیاز به ایجاد دستورالعمل‌های حرفه‌ای و كدهای اخلاقی برای تجربیات داروخانه‌ای در راستای پوشش دادن همه‌ی مسائل اخلاقی است

Pharmacists serve as members of both the society and the health care team, which may involve them in activities that could be the subject of ethical query. They can face issues and problems in the workplace that may raise questions as to whether their acts are appropriate, right, just or legal. This study aims to explore ethical issues from the perspective of pharmacists and to examine their reasoning processes regarding social phenomena such as ethical challenges. Moreover, it explores the ethical decision-making strategies that pharmacists employ when dealing with such problems in pharmacies and in their interactions with people. This was a qualitative content analysis study. The participants were chosen using purposeful sampling from community pharmacists and the academic staff of the School of Pharmacy at Shiraz University of Medical Sciences. The data were obtained through semi-structured interviews with 20 individuals and analyzed using content analysis approach. Analysis of the interviews led to extraction of the following six categories: pharmacistsʹ understanding of key ethical concepts; pharmacistsʹ experiences and duties; relationship with other health professionals; the pharmacy as a commercial setting; the difference between the practice of ethics in pharmacy, medicine and nursing from the perspective of pharmacists; and the actual response of pharmacists in confronting ethical problems.  Each of the above-mentioned categories were further divided into sub-classifications describing specific ethical challenges in pharmacy practice Awareness of ethical issues and professional guidelines is a fundamental part of the education of health care professionals and provides a structure that will enable them to assess ethical problems and make appropriate, justifiable decisions. The findings of this study indicate the need for implementation of professional guidelines and codes of ethics in pharmacy practice to cover all ethical issues
