ميزان تأثير سطح تحصيلات بهورزان بر كنترل فشار خون مبتلايان به فشار خون بالا در روستاييان شهرستان قزوين

The effect of health behvarz’ education level on blood pressure control among hypertensive patients in the rural areas of Qazvin city, Iran

زمینه و هدف: پرفشاری خون یكی از مهمترین عوامل خطر ساز بیماری های قلبی عروقی است.كنترل فشار خون احتمال ایجاد عوارض ناشی از فشار خون بالا را به حداقل مقدار خود می رساند. هدف از انجام این مطالعه تعیین تأثیر سطح تحصیلات بهورزان بر كنترل فشار خون در مبتلایان به فشار خون بالا در روستاییان شهرستان قزوین بود. روش كار: در این مطالعه مقطعی كه بر روی  بیماران مبتلا به فشارخون بالا و بهورزان خانه های بهداشت شهرستان قزوین در سال 1389 انجام شد،300 بیمار و 18 بهورز در مطالعه شركت كردند. نمونه گیری با روش چندمرحله ای انجام شد. ابزارهای جمع آوری داده­ها پرسشنامه، دستگاه فشار سنج جیوه ای،گوشی، ترازوی دیجیتالی و قدسنج دیواری بود. روش جمع آوری داده ها مصاحبه و مشاهده بود. داده‌ها با استفاده از نرم‌افزارSPSS و آزمون‌آماریChi Square  تحلیل شد. نتایج: شیوع بیماری فشارخون بالا در جمعیت تحت پوشش خانه های بهداشت دارای بهورزان دیپلمه 2/13% و در جمعیت تحت پوشش خانه های بهداشت دارای بهورزان زیر دیپلم 7/8% بود. فقط 7/14% بیماران بررسی شده فشار خون كنترل شده داشتند. میزان كنترل فشار خون بیماران در دو گروه بهورزان دیپلمه و زیر دیپلم از نظر آماری معنی دار نبود. نتیجه گیری: با توجه به عدم تأثیر سطح تحصیلات بهورزان بر كنترل فشارخون، پیشنهاد می­شود در برنامه ریزی های بهداشتی به سواد كاركنان بهداشتی محیطی توجه بیشتری شود و به آموزش ضمن خدمت پرسنل و نظارت های منظم و بررسی شاخص های بهداشتی تأكید گردد.

Background and Aim: Hypertension is a major risk factor for cardiovascular diseases. Its control decreases the risk of likely complications. The aim of this study was to determine the effect of behvarz’ education level on blood pressure control among hypertensive patients in the rural areas of Qazvin city, Iran. Materials and Methods: This cross-sectional study, including 300 hypertensive patients and 18 behvarz selected by multi-stage sampling, was conducted in health houses of the rural areas of Qazvin city in 2010. Usning standard methods and technics, weight, height and blood pressure were measured. Data were collected by interviews (using questionnaires) and observations and anlyzed using the SPSS software and the Chi Square test as the statistical test (level of sinnificance 0.05 ). Results: The prevalence of hypertension was 13.2% and 8.7% in the populations under coverage of the health houses manned by community behvarz with a high school diploma and those with an education level below high school diploma, repectively. Only 14.7% of the patienes studied had their blood pressure controlled. There was no statistically significant association between the extent of patients’ blood pressure control and behvarz’ educationl level. Coclusion: It can be concluded that education level of the behvarz has no effect on hypertensive patientsʹ blood pressure control. In any case, it is recommended to pay more attention to the education and literacy of peripheral health workers and put emphasis on the personnel in-service training, regular monitoring and supervision, as well as health indices assessment.
