Prioritization of Strategic Factors Influencing Health Care Management with a Hybrid Approach: A Case Study in Public Hospitals of Rasht, Iran

مقدمه: دسترسي عادلانه به خدمات سلامت، حق همه افراد جامعه مي‌باشد و دولت‌ها بايد اين خدمت را در اختيار مردم قرار دهند. هدف از انجام اين مطالعه، اولويت‌بندي عوامل موثر استراتژيك مديريت سلامت با استفاده از رويكرد پيوندي در بيمارستان‌هاي دولتي بود. روش بررسي: اين تحقيق به روش توصيفي- پيمايشي انجام شد و از لحاظ هدف، كاربردي بود. شش بيمارستان دولتي شهر رشت در سال 1393 مورد مطالعه قرار گرفت. اين پژوهش بر اساس رويكرد پيوندي شامل تركيب فرايند تحليل شبكه‌اي فازي (Fuzzy network analysis process ( Fuzzy ANP براي اولويت‌بندي عوامل موثر استراتژيك مديريت سلامت و ديمتل فازي (Fuzzy Decision Making Trial And Evaluation ( Fuzzy DEMATEL جهت به دست آوردن شدت تاثير عوامل استراتژيك مديريت سلامت بود و با استفاده از تاپسيس فازي Fuzzy Technique For Order Preference By Similarity To Ideal Solution ( Fuzzy TOPSIS بيمارستان‌ها را رتبه‌بندي كرد. ابزار جمع‌آوري داده‌ها، پرسشنامه بود كه در شش بيمارستان دولتي مورد ارزيابي قرار گرفت. يافته‌ها: معيارهاي مربوط به تحليل نقاط قوت‌، نقاط ضعف‌، فرصت‌ها و تهديدهاي Strengths, Weaknesses, Opportunities, Threats يا SWOT به كار رفته در مطالعه حاضر داراي 4 معيار اصلي و 16 زيرمعيار بود كه رتبه‌بندي گرديد. در بين شش بيمارستان دولتي، بيمارستان پورسينا رتبه اول و بيمارستان‌هاي رسول اكرم (ص)، اميرالمومنين (ع)، حشمت، 17 شهريور و رازي به ترتيب در رتبه‌هاي بعدي قرار گرفتند. معيارها و زيرمعيارهاي تحقيق، هر كدام رتبه‌اي را به خود اختصاص دادند كه نشان‌ دهنده اهميت آن‌ها در بخش بهداشت و درمان بود. نتيجه‌گيري: نتايج مطالعه حاضر با مشخص نمودن اهميت هر يك از معيارها و زيرمعيارهاي مربوط به تحليل (SWOT)، به بهتر شدن عملكرد مديريت سلامت كمك نمود. واژه هاي كليدي: مديريت سلامت؛ تحليل استراتژيك؛ بيمارستان‌هاي دولتي

Abstract Introduction: Fair access to health care services is every individual’s right in society and governments should provide these services. The purpose of the present study was to prioritize strategic factors influencing health care management using a hybrid approach in the context of public hospitals. Methods: The current research was an applied, descriptive survey. It was based on a hybrid approach including a combination of Fuzzy Analytic Network Process (ANP) for prioritization of strategic factors influencing health care management and Fuzzy Decision Making Trial and Evaluation Laboratory (DEMATEL) for identification of the extent of the effects of strategic factors of health care management. In addition, the studied hospitals were ranked using the Fuzzy Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS). Questionnaires were used for collection of data from 6 public hospitals in 2014. Results: SWOT (strengths, weaknesses, opportunities, and threats) analysis criteria used in the present research comprised of 4 main criteria and 16 sub-criteria which were ranked in the present study. Among the 6 public hospitals studied in the present study, Poursina Hospital obtained the first rank and Rasoul Akram, Amir-al-Mo’menin, Heshmat, Hefda Shahrivar, and Razi Hopitals obtained the following ranks, respectively. Each of the Criteria and sub-criteria obtained a rank based on their importance in the health care sector. Conclusion: The results of the present research can contribute to the improvement of the performance of health care management by determining the importance of the criteria and sub-criteria associated with SWOT analysis.
