بررسي ديدگاه كادر درماني و آموزشي بيمارستان افضلي پور كرمان نسبت به سيستم پشتيبان تصميم گيري باليني طراحي شده به منظور كاهش احتمال بروز آمبولي ريه و ترومبوز وريدي عمقي

Surveying Kerman’s Afazalipour Hospital clinical and educational staff`s points of view about the clinical decision support system designed for reducing the possibility of pulmonary embolism and deep vein thrombosis

زمینه و هدف : این پژوهش در نظر دارد تا یك نرم افزار كه قابلیت تشخیص سریع افراد مستعد به بروز آمبولی ریه و ترومبوز وریدی عمقی پا به همراه امكان ارسال یاد آور به موقع جهت انجام عملیات پیشگیری از بروز این موارد را داشته باشد را طراحی و پیاده سازی كند. هدف نهایی معرفی سیستم پشتیبان تصمیم گیری است كه در نهایت بتواند با انجام عملیات مناسب از بروز موارد معلولیت و مرگ و میر ناشی از آمبولی و ترومبوز در بیماران بستری در بخشهای مرتبط جلوگیری نماید. مواد و روش ها : طراحی این نرم افزار با زبان برنامه نویسی Visual Basic.Net و دیتابیس SQL Server صورت گرفت. سپس این نرم افزار در بزرگترین بیمارستان آموزشی شهر كرمان نصب و یك ارزیابی و نظرسنجی از پزشكان و پرستاران درباره كاركرد نرم افزار با استفاده از پرسشنامه و مصاحبه های مختلف انجام شد. سپس اطلاعات توسط نرم افزار Spss آنالیز گردید. یافته ها : میانگین نمرات پزشكان 21.16 و پرستاران 20.76 بدست آمد. نتایج آزمون T-Testنشان می دهد كه بین میانگین كل نمرات پزشكان و پرستاران اختلاف معنی داری وجود ندارد. نتیجه گیری : نتایج حاصله نشان داد كه هر دو گروه پزشكان و پرستاران نسبت به نرم افزار، نظر مساعد و مثبتی داشتند. بنابراین استفاده از سیستم پشتیبان تصمیم گیری بالینی كه پیشگیری را با دادن هشدارهای سروقت به كادر درمانی یادآوری می كنند، می تواند در كاهش بروز آمبولی ریه و ترمبوز وریدهای عمقی مؤثر و سودمند باشد.

Background: This research aims to design and implement a software with the ability to identify patients who are facing the risk of pulmonary embolism and deep venous thrombosis instantly as well as the ability to send timely reminders for any prophylactic action. The main target is introduce a clinical decision- support system which could finally lead to preventing mortality and handicap cases caused by embolism and thromboses in patients who are confined to bed in hospitals. Materials and Methods: The software was designed using the Visual Basic.Net and SQL Server database. Afterwards the software was installed in the largest educational hospital of Kerman and a survey was conducted amongst the physicians using multiple questionnaires and interviews. Finally, the data were analyzed using the SPSS software. Results: The average score was 21.16 for the physicians and 20.76 for the nurses. T-Test results show that there is no significant difference between the total average score of the physicians and that of the nurses. Conclusion: The results have shown that both groups (physicians and nurses) have a positive viewpoint about the software therefore using the clinical decision support system can be effective in reducing the occurrence of pulmonary embolism and deep venous thrombosis through sending timely electronic alerts to the medical staff.
