بررسي ميزان رعايت منشور حقوق بيمار از نظر كارورزان در بيمارستان‌هاي آموزشي دانشگاه علوم پزشكي شهيد بهشتي

Assessment of medical students’ opinion about the patients’ bill of rights observance in the educational hospitals of Shahid Beheshti University of Medical Sciences

سابقه و هدف: رعايت حقوق بيماران موجب بهبود ارتباط بين بيماران و كاركنان مراكز درماني و ارتقاي كيفيت خدمات سلامت مي‌گردد. اين مطالعه جهت بررسي ميزان رعايت منشور حقوق بيمار از نظر كارورزان در بيمارستان‌هاي دانشگاه علوم پزشكي شهيد بهشتي انجام شده است. مواد و روش‌ها: در اين پژوهش توصيفي- مقطعي، 137 نفر از كارورزان رشته پزشكي از بيمارستان‌هاي امام حسين (ع)، مدرس، طالقاني، لقمان، مفيد، شهداي تجريش، لبافي نژاد و مهديه به روش نمونه‌گيري در دسترس انتخاب شدند. ابزار جمع‌آوري داده‌ها پرسشنامه 12 سوالي پژوهشگر ساخته بود كه روايي آن توسط اساتيد رشته و پايايي آن با ضريب كرونباخ آلفاي 81 درصد تاييد شد. داده‌ها با استفاده از روش‌هاي آمار توصيفي و آزمون آناليز واريانس و آزمون Tukey توسط نرم افزار SPSS (نسخه 21) مورد تجزيه و تحليل قرار گرفت. يافته‌ها: ميزان رعايت منشور حقوق بيمار در بين تمام بيمارستان‌ها در 6/41% موارد مطلوب، در 5/55% موارد متوسط و در 9/2% موارد نامطلوب بود. بيشترين ميزان رعايت منشور حقوق بيمار مربوط به "امكان ترخيص بيمار با رضايت شخصي" (5/92%) و كمترين ميزان مربوط به "مشاركت بيمار در انتخاب روش درماني" (2/46%) بود. بين بيمارستان‌هاي مختلف از نظر رعايت منشور حقوق بيمار تفاوت معني‌داري مشاهده شد (013/0P=). نتيجه‌گيري: ميزان رعايت منشور حقوق بيماران در بيشتر بيمارستان‌هاي دانشگاه علوم پزشكي شهيد بهشتي در حد متوسط تا مطلوب است. با اين وجود، ميزان رعايت اين حقوق در بعضي بيمارستان‌ها در حد مطلوب نيست. لذا توصيه مي‌شود برنامه‌هاي آموزشي جهت آشنايي كاركنان و بيماران با منشور حقوق بيمار اجرا شود.

Background and Aim: Respecting patients’ rights leads in improvement of communication between patient and health care providers and promotion of health services. This study is performed to assess the patientsʹ bill of rights observance in the educational hospitals of Shahid Beheshti University of Medical Sciences. Materials and Methods: In this descriptive-cross sectional study 137 medical students (interns) were selected form Imam Hosein, Modarres, Taleqani, Mofid, Shohadaye Tajrish, Labbafinejad and Mahdie hospitals by convenience sampling method. We used a questionnaire with 12 questions. Experts of the field confirmed reliability of questionnaire and validity was confirmed by Cronbach’s Alpha coefficient (81%). The obtained data were analyzed by SPSS (v21) using descriptive statistics, analysis of variance and Tukey test. Results: In this study the observance rate of patients’ bill of rights was at a good level in 41.6% of cases, at an average level in 55.5% of cases and at a low level in 2.9% of cases. The best observance rate was for the article “discharge with informed consent” (92.5%) and the lowest observance rate was for the article “participation of patient in selecting treatment alternatives” (46.2%). There was a significant difference between several hospitals in the observance rate of patients’ bill of rights. (p-value = 0.013). ‍Conclusion: The observance rate of patients’ bill of rights was at an average to good level in most of Shahid Beheshti University of Medical Sciences’ hospitals. However, this observance rate is at a low level in some hospitals. Therefore, it is recommended to design and implement educational programs for personnel and patients to promote their awareness about patient rights.
