بررسي فراواني، موقعيت و ابعاد سپتاهاي سينوس ماگزيلاري بر روي تصاويرسي‌تي‌اسكن با اشعه مخروطيافراد بالغ در شهر اصفهان

Frequency,location and size of maxillary sinus septa on CBCT images in adult patients in Isfahan

مقدمه: وجود سپتاهاي سينوس ماگزيلاري يكيازمشكلات مهم درفرايند سينوس ليفت وافزايش سوراخ شدن (Perforation)غشاي سينوس است. هدف از اين مطالعه تعيين فراواني،موقعيت وابعاد سپتاهاي سينوس ماگزيلاري بر روي تصاوير(CBCT) Cone Beam Computed Tomographyافراد بالغ بود. مواد و روش‌ها: دراين مطالعه توصيفي- تحليلي، تصاوير CBCT از 75 بيمار بالاي 18 سال مورد بررسي قرار گرفت سپسفراواني،محل وابعاد سپتاهاي سينوس ماگزيلاري بررسي شد. 75 بيماردرسه گروه 25 نفري برحسب تعداد دندان‌ها تقسيم شدند:بي‌دنداني كامل خلفي،بي‌دنداني پارسيل خلفي وافراد با سيستم دنداني كامل خلفي.تصاوير ازلحاظ موقعيت،سپتاها درسهناحيه بررسيشدند:ناحيه قدامي(منطقه پره‌مولرها)، مياني(ديستال پره‌مولردوم تا ديستالمولردوم) و خلفي (ديستال مولردوم).طي اين مطالعه محل و ابعاد سپتا در نرم‌افزار Ondemand 3D مورد بررسي قرار گرفت. داده‌ها با استفاده ازآزمون‌هايواريانس، مجذوركاي، مك‌نمار و كروسكال‌واليس مورد تجزيه و تحليل قرار گرفتند (05/0 =?). يافته‌ها: در تصاوير CBCT از 150 سينوس ماگزيلاري، 86 سپتا (3/57%) ديده شد. بيشتر سپتاها (53%) در منطقه مياني سينوس ماگزيلاري وجود داشتند. فراواني سپتاهاي سينوس در افراد بي‌دنداني كامل خلفي (30%)، در افراد بي‌دندان پارسيل خلفي (37%) و در افراد با سيستم دنداني كامل خلفي (33%) بود كه اختلاف معني‌داري بين سه گروه مورد مطالعه وجود نداشت (37/0 = p-value) همچنين ميانگين ارتفاع سپتا بطور كلي (6/3 9/5) ميلي‌متر گزارش شد. نتيجه‌گيري: بيشتر سپتاها در ناحيه مياني سينوس بودند ولي بين سه گروه دنداني اختلاف آماري معني‌داري وجود نداشت، ازآنجايي كهتنوعات وسيع آناتوميك در فراواني، سايز، موقعيت و مورفولوژي سپتاي سينوس وجود داشت لذا هنگام سينوس ليفت تصويربرداري CBCTمفيد خواهد بود. كليد واژه‌ها: سپتا،سينوس ماگزيلاري، سي‌تي‌اسكن با اشعه مخروطي

Introduction: The maxillary sinus septa area majorproblem insinus lift procedures, resulting in an increase insinusmembraneperforation. The aim of thisstudy was to determinethe frequency, location and size of maxillary sinus septa on CBCT images in an adult population. Materials & Methods: In this descriptive-analytical study, CBCT images of 75 patients > 18years of age were evaluated. Then the frequency, location and size of maxillary sinus septa were determined. The patients were divided intothree groups (n=25) based on the number of teeth: posterior complete edentulism, posterior partial edentulism and posterior complete dentition. The images were evaluated in terms of the location of septa in three areas: the anterior (premolar region), middle (distal to the second premolar to distal to the second molar) and posterior (distal to the second molar). The septa sizes and locations were evaluated using Ondemand 3D software program. Data were analyzed withANOVA and chi-squared, McNemar andKruskal-Wallis tests (? = 0.05). Results: In 150 maxillary sinuses, CBCT images showed 86 septa (57.3%). Most septa (53%) were located in the middle region of maxillary sinuses. Sinus septa were present in 30% of posterior totally edentulous subjects, 37% of posterior partially edentulous subjects and 33% of patients dentate in the posterior region, with no significantdifferences betweenthethree groups (p value = 0.37). Themean heightof thesepta was 5.9±3.6 mm. Conclusion: Most septa were located in the middle region of maxillary sinuses, with no statistically significantdifferences betweenthethree groups. CBCT might provide useful information during sinus lift procedures, considering wide anatomic variations in the frequency, size, location and morphology of maxillary sinus septa.
