تأثير مداخله پرستاري مبتني بر بكارگيري شوخي بر افسردگي نوجوانان مبتلا به لوسمي

The impact of nursing interventions based on the use of humor on depression in adolescents with leukemia

مقدمه:افسردگي بعنوان يكي از عوارض جدي سرطان تأثيرات متعدد و سويي در زمينه هاي مختلف زندگي فردي و اجتماعي، سلامت رواني و جسماني، درمان و پيشرفت بيماري فرد برجاي ميگذارد. مطالعه حاضر با هدف تعيين تأثير مداخله پرستاري مبتني بر بكارگيري شوخي بر افسردگي نوجوانان مبتلا به لوسمي انجام شده است. روش:پژوهش حاضر يك مطالعه مداخله كارآزمايي باليني است كه در آن 62 كودك و نوجوان 12 تا 18 ساله مبتلا به لوسمي كه معيارهاي ورود به مطالعه را دارا بودند به روش هدفمند انتخاب و بصورت تصادفي در دو گروه مداخله و شاهد تخصيص داده شدند. جهت گردآوري داده ها از پرسشنامه هاي جمعيت شناختي و افسردگي كودكان و نوجوانان (CADS) استفاده گرديد. در گروه مداخله از مداخله پرستاري مبتني بر بكارگيري شوخي بصورت 3 جلسه 30 دقيقه اي در هفته، به مدت 8 هفته استفاده گرديد و در گروه شاهد تنها مداخلات روتين بيمارستان انجام شد. قبل و بعد از اتمام مداخله هر دو گروه پرسشنامه هاي مربوطه را پر كردند. يافته ها:مابين متغيرهاي جمعيت شناختي در دو گروه اختلاف آماري معني داري پيدا نشد (05/0 p). ميانگين نمره افسردگي در گروه مداخله در پيش آزمون 16/36 بود كه بعد از مداخله در پس آزمون به 19/18 رسيد. آزمون آماري تي زوجي اختلاف معنادار آماري مابين تفاضل نمرات قبل و بعد نشان داد (001/0>p). نتيجه گيري:نتايج مطالعه حاضر نشان داد كه بكار بردن مداخلات پرستاري مبتني بر شوخي مي تواند تا حد زيادي از افسردگي كودكان و نوجوانان مبتلا به لوسمي بكاهد و بعنوان يك روش مراقبتي مؤثر در مراقبت از اين بيماران توصيه مي گردد.

The impact of nursing interventions based on the use of humor on depression in adolescents with leukemia Abstract Introduction:Depression as a serious complication of cancer has various effects in different contexts like on the individual and social life, mental and physical health, treatment and disease progression. This study aimes to determine the effect of nursing intervention based on humor on depression in adolescents with leukemia. Method:This researchis an experimental studywhere 62 children and youth aged 12 to 18 years old with leukemia that have inclusion criteria were that purposive sampling and were randomly assigned into two intervention and control groups. To gather the data, demographic and depression in children and adolescents (CADS) questionnaires were used. In the experimental group, nursing interventions based on the use of humor as three 30minute sessions per week, for 8 weeks were used and control group only received the routine care interventions. Before and after intervention, both groups filled out questionnaires. After collecting the data entered and analyzed using SPSS version 21. Findings:Statistically significant difference was not found in demographic variables between the two groups (P>0.05). Pretest mean depression score in the control group was (34.10&±;0.16) and the experimental group was (36.16&±;6.89), which statistically significant difference wasn&rsquo;t found between the two groups in pretest scores (P>0.05). After adjusting pretest scores, a significant effect of depression between subjects in grades obtained (P<0.001). The average depression score in the experimental group in pretest was 36.16 which was 18.19 after the intervention ended. Paired ttest showed significant differences between the scores before and after (P<0.0001). Conclusion: Results of this study showed that use of nursing interventions based on the humor can greatly reduce depression in children and adolescents with leukemia and as a method of health care in the care of these patients was recommended.
