Review the Opinions of Physicians in the Country Concerning the Causes of Diagnostic Medical Errors

براي كاهش ميزان خطاهاي تشخيصي در حيطه پزشكي، شناخت علل بروز و نگاه عوامل بروز خطاي انساني بسيار حائز اهميت است؛ چراكه ميزان آگاهي آنها از علل واقعي بروز اين خطاها را مي‌رساند. اينكه خود پزشكان نسبت به علل بروز اين خطاها چه نظري دارند، بيانگر ميزان شناخت آن‌ها از مهمترين علل بروز خطاهاي تشخيصي است. در گام بعدي جاي تأمل دارد كه آيا اين ديدگاه واقع بينانه است يا نياز به تغيير نگرش دارد؟ اين مقالۀ پژوهشي و با مطالعه كتابخانه‌اي- اسنادي و ميا‌نرشته‌اي است كه در تعامل با 50 نفر از پزشكان رشته‌هاي مختلف در سطح كشور به صورت مطالعه كيفي و با استفاده از پرسشنامه 5 طيفي خطاهاي تشخيصي پزشكي به صورت خودساخته جمع‌آوري شده است و پس از آن پرسشنامه 5 طيفي، با اولويت بالاترين خطاهاي تشخيصي مطرح از نگاه پزشكان تهيه شده است. نمونه گيري، به صورت مبتني بر هدف بود و مصاحبه‌شوندگان داراي حداقل سه سال سابقه كار بودند. اين مطالعه پس از طرح در گروه متمركز، اصلاح شد و به صورت باز پاسخ مجدد در اختيار گروه قرار گرفت. از نگاه پزشكان بيشترين علت بروز خطاهاي تشخيصي پزشكي مربوط به همكاري نكردن بيمار در ارائه اطلاعات صحيح و كامل و همچنين خطاي اعتبارسنجي نكردن داده‌ها با شرايط بيمار است. دانش ناكافي معاينه‌كننده نسبت به وضعيتِ موجود بيمار، خطاي نتيجه‌گيري نكردن مناسب از داده‌هاي معاينه در تشخيص، خطاهايي كه با وجود آگاهي پزشك از وجود آن خطا، با تمركز او بر تنها علامت خاصي از درك بقيه علائم خود را محروم مي‌كند و خطاي درك نكردن علائم به‌شكل صحيح و ناديده گرفتن و خطاي خاتمه زودرس از ديگر خطاهاي معمول هستند.

Medical errors, including medical diagnostic errors, are one of the most significant issues in the world’s health system, and many of these errors will lead to judicial follow-up. In order to reduce the level of diagnostic errors in the medical field, recognizing the causes of the occurrence and appearance of the causes of human error is very important because their level of awareness is due to the true causes of these errors. What doctors themselves think about the causes of these errors indicates their degree of recognition of the most important causes of diagnostic errors. The next step is to reflect on whether this view is realistic or needs to change attitudes. This article is a research paper with a library and documentary and interdisciplinary study that was conducted in collaboration with 50 different physicians in the country in a qualitative study using a 5-spectrum diagnostic medical errors questionnaire. The questionnaire has been developed by a self-developed person and then a 5-point questionnaire with the highest priority of diagnostic errors from physicians’ point of view. Sampling is based on goal and interviewees have at least three years of work history. This post-design study has been modified in the centralized group, and has been represented to the group. From the physicians’ point of view, the most common cause of medical diagnostic errors is the lack of cooperation of the patient in providing accurate and complete information, as well as the lack of validation of the data with the patient’s condition. Examiner’s inadequate knowledge about the current status of the patient, proper data error, despite the knowledge of the physician about the error, he/she stops him/herself from adequate diagnosis by focusing too much on one special symptom, ignoring the error and not understanding the right symptoms and the premature closure, are the other errors.
